Pharmacy

The mission of the pharmacy service is to enhance patient safety and outcomes through delivery of the highest quality, evidence-based medication therapy services and education to Millcreek Health System and the community. The pharmacy service, in collaboration with other medical professionals, is responsible for all medication safety and medication use policy and procedure within the health system.

Our hospital pharmacists are part of a multidisciplinary team that reviews the medication regimen of each patient upon admission and as they move through the hospital. This medication reconciliation process is in place to ensure that patients get maximum benefit from the medications they receive with no adverse effects.

Our patients will also receive pharmacy service provided medication counseling on the prescription and over-the-counter medications patients will be taking upon discharge from our hospital. Our pharmacists will high-light the important points regarding these medications and ensure the patient has a clear understanding of the information that has been provided.

In addition to medication counseling, the pharmacy service also performs adult immunization screening. Adult immunization against pneumococcal pneumonia and the influenza virus has been shown to significantly reduce illness and hospitalization from these infections. Smoking cessation counseling services are also provided and available to all of our patients.

Millcreek Community Hospital and Millcreek Manor are unique in its delivery of pharmacy services to our patients and residents. As teaching facilities affiliated with LECOM School of Pharmacy, students at all levels of their education rotate at the hospital. They learn and practice patient-centered care and provide pharmacy services to our patients and residents under the direct supervision of a registered pharmacist. Mentoring pharmacy students in this manner not only allows us to give more individualized pharmaceutical care to our patients now, it also trains the next generation of pharmacists who will care for you and your family in the future.

What you need to know

Millcreek Community Hospital Pharmacy is a full service pharmacy providing a complete range of safe and effective medications for all inpatients, outpatients and long-term care residents.

Our hours of operation are Monday through Friday from 7:30 a.m. to 11:00 p.m. and Saturday, Sunday and holidays from 8:00 a.m. to 4:30 p.m. A Millcreek Community Hospital pharmacist is on-call 24 hours a day, seven days a week to provide emergency medication therapy or drug information to all patients, residents and health care professionals without delay. Our pharmacists are available to answer any medication-related questions patients or families may have after discharge.
