2nd Shift Home Health RN Case Manager / Registered Nurse

The Visiting Nurse Association of Erie County, part of the LECOM Institute for Successful Aging, is currently seeking Full Time 2nd Shift RN Case Manager to join our team. The VNA serves patients in both Erie and Crawford counties.

The schedule for this position will consist primarily of 2nd shift hours, possibly (3) 12 hr shifts (11am-11pm), (4) 10 hr shifts, or something similar, including some weekend hours. Schedule flexibility is also required, based on the needs of the patients.

In addition, we are now offering: **$5,000 to $12,000 Sign on bonus for Full Time Direct Patient Care Registered Nurse Case Managers with opportunity to earn up to $80K in total bonuses!**


To provide nursing care in accordance with the patient’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention, coordination of services, teaching and training activities and direct nursing care.


Education and experience equivalent to:

  • Must be a graduate from an accredited School of Nursing
  • Must be licensed in Pennsylvania as a Registered Nurse
  • One or more years of experience in community/home health agency or in a hospital setting is preferred.
  • One or more years of RN experience is preferred.
  • Must have valid driver’s license and reliable transportation.
