Tri-State Elder Assessment Management (T.E.A.M.)

The LECOM Tri-State Elder Assessment Management (T.E.A.M) program specializes in providing mobile assessments for individuals age 50 and older who are experiencing a change in mental status, behavior or mood. Our knowledgeable and compassionate staff work individually with each patient to assess and identify their personal needs. Assistance is then provided to actively coordinate appropriate treatment to meet the needs of the older adult patient.

T.E.A.M assessments are provided at no cost and can be completed in a familiar setting at the convenience of you and your loved one. If you or a loved one are experiencing any of the symptoms listed below, please contact us to schedule an assessment. We are here to help.

  • Severe depression or anxiety
  • Thoughts of harm to self or others
  • Increased confusion
  • Changes in sleeping and/or eating habits
  • Inability or unwillingness to complete necessary self-care tasks
  • Agitation or aggression
  • Thought or mood disturbance
  • Hallucinations or delusions

The availability to schedule assessment by the (T.E.A.M) may be contingent on current protocols to mitigate the risk of contagions such as in the case of COVID-19.
