Living Will

You have the right to decide the type of health care you receive. Should you become unable to understand, make, or communicate decisions about medical care, your wishes for medical treatment are most likely to be followed if you express those wishes in advance.

The Pennsylvania Department of Health provides a sample Living Will document, which is legal in Pennsylvania to insure that your wishes are followed.

You may complete this form as your Living Will, or you may use it to assist you in developing a Living Will that best communicates your own wishes.
