Healthy Employees Build Resilient Organizations

LECOM Health HERO Logo

HERO is a program established by LECOM Health as a part of the Pennsylvania Department of Health LTC RISE Program (Long-Term Care Resiliency, Infrastructure Supports, and Empowerment). In addition to supporting the COVID-19 outbreak response in long-term care facilities (LTCF), HERO will enhance the quality of care and the ability to prevent and respond to disease outbreaks, including COVID-19 in Northwest Pennsylvania’s long-term care facilities by:

  • Promoting professional development: increasing protective factors in resilience, wellness, and mental health; decreasing stress and risk of burnout among health professionals, and using the evidence-supported, rapid deployment of training, resources, and support to LTCF staff, teams, and leaders.
  • Supporting the journey to become age-friendly health systems by aligning infection prevention and control and emergency preparedness efforts with best practices.
  • Building a sustainable outbreak response operation using available resources in the community and region that can be activated when necessary to mitigate future infectious disease outbreaks.
  • HERO Counties Served: Cameron, Clarion, Clearfield, Crawford, Elk, Erie, Forest, Jefferson, Lawrence, McKean, Mercer, Venango, and Warren

Enroll for LTC RISE 4.0

To enroll in the LTC RISE 4.0 grant program, a new LTC RISE Needs and Readiness Assessment must be completed. Please note: a separate assessment will need to be completed for each license you have, even if they are at the same location (e.g., your SNF, PCH, and/or AL/IL would need a separate assessment completed.) Please reach out to your LTC RISE partner if you have any questions.
