An Activity Aide will assist in organizing, directing and carrying out programs and activities which cover spiritual development, recreation, diversion, intellectual, and educational development for residents as an integral part of resident centered care.
JOB TASKS AND RESPONSIBILITIES:
- Knowledge, skills, attitude, and energy to consistently provide resident centered care.
- Organizes and directs or carries out a program of activities which will provide entertainment, intercommunication, relaxation, opportunity to express creative talent and fulfill basic psychological, social and spiritual needs for all the residents, subject to individual restrictions, limitations, and interests.
- Encourages hobbies and craft activities. Provides craft materials and supplies, instructs and supervises residents during craft activities, and complies with safety regulations and policies.
- Encourages social communication among residents through group activities and recreation.
- Helps in planning parties, provides games and encourages residents to assist in the planning.
- Encourages residents to participate in religious activities to fulfill their spiritual needs.
- Encourages intellectual/educational development through literature, lectures and cultural events.
- Provides reading material and CDs, tapes and digital books for the blind.
- Assist in recording of residents’ activities. Assists in admissions, evaluations and MDS process. Writes progress notes as needed.
- Must perform as a Universal Worker being cross-trained in housekeeping, dietary, and personal care functions.
- Must be flexible to accept other duties as needed or assigned by the resident or institution.
- Performs other duties as assigned by supervisor or management.
MINIMUM QUALIFICATIONS:
- High School Diploma, GED, or equivalent is preferred
- Previous customer service or personal care experience is preferred
- Must be willing to undergo a pre-employment physical and criminal background check

Position Summary
The Personal Care Home Administrator is responsible for the overall management and daily operations of the facility. This includes supervision of staff, regulatory compliance, financial oversight, resident care, and fostering a positive and supportive environment for residents, families, and employees.
Key Responsibilities
- Ensure compliance with Pennsylvania Department of Human Services (DHS) Personal Care Home regulations (55 Pa. Code Chapter 2600).
- Provide leadership and direction to all staff, promoting a culture of compassion, teamwork, and accountability.
- Oversee resident admissions, assessments, and care plans.
- Maintain accurate records, reports, and documentation as required by law and company policy.
- Manage budgets, payroll, and financial reporting in collaboration with ownership/management.
- Coordinate staff training, scheduling, and performance evaluations.
- Serve as the primary liaison between residents, families, healthcare professionals, and regulatory agencies.
- Implement and monitor policies to ensure high-quality care and resident satisfaction.
Qualifications
Required:
- Current Personal Care Home Administrator Certification issued by the Pennsylvania Department of Human Services.
- Minimum of 2 years of experience in a supervisory or administrative role within a personal care or long-term care setting.
- Strong working knowledge of PA DHS personal care regulations.
- Excellent communication, leadership, and organizational skills.
Preferred:
- Bachelor’s degree in healthcare administration, human services, or related field.
- Experience managing budgets and overseeing staff development.

Job Summary:
To register patients throughout all departments of Corry Memorial Hospital in compliance with the LECOM Health Corry Memorial Hospital policies and procedures.

- Register patients and update existing patient demographics by collecting detailed information including personal and financial information along with correct system information.
- Confirm and verify insurance benefits with the appropriate carrier via eligibility verification and/or appropriate insurance websites.
- Scan insurance card, driver’s license and order(s).
- Print face sheet(s), labels and apply arm bands on patients.
- Validate physician’s order for all appropriate information.
- Contact physician’s office if there is no order on file/or need new standing order.
- Collect any co-payments due.
- Complete all applicable forms and necessary communications while obtaining signature for consent to treatment and financial responsibility, HIPPA and Photo Consents.
- Notify departments of patient’s arrival.
- Direct patients to appropriate department for testing.
- File orders in drawer alphabetically as they come in from fax machine.
- Purge orders from drawer that are over 6 months old; verify if testing was done; send to physician’s office to let them know patient did have done at our facility.
- Follow up calls made to Physician’s offices if information not received.
- Look up Physician information if ordering physician is not in the system and add it; Give information to Special Projects Personnel.
- Late Add Ons are registered if not done by pre-reg/insurance verification.
- Greet, interview triage, and register ER patients; including obtaining all signatures, updating demographical information, and verifying insurance eligibility.
- Perform Cashier functions by collecting co-payments, counting cash drawer, providing change.
- Assist with outpatient registration, obtaining medical records, all intercoms, switch board after departmental business hours, and paging of physicians.
- Provide Clerical Assistance for the Emergency Department by scanning, filing, obtaining records, and completing tear down chart
- Distribute OBS Brochures as necessary.
- Assist Emergency Room Nurse and Physician as requested; such as assisting patients per RN or Physician instruction, obtaining wheelchairs, collecting urine specimens, cleaning beds, and copying charts for transfers.
- Assist Ambulance Service as needed; such as answering phone, open/close garage door, meet as they arrive, answer Ambulance Med patch.
- Set up arrangements for transfer by calling Transfer Center as well as helicopter transports.
- Monitor panel alarms and security cameras.
- Announce Fire Alarms.
- Notify departments of Cardiac Arrest.
- Contact on call Administrative personnel as needed.
- Assist in finding nursing staff coverage when needed.
- Complete transfer sheets and daily counts for Administration office.
- Assist Acute Care Unit with scanning living wills and POA’s.
- Verify room and bed census with nursing floors nightly.
- Call outside services such as Poison Control and Chaplain as needed.
- Snow removal and/or salting when needed.
- Assist with other tasks as assigned by Revenue Cycle Manager.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Review physician orders, patient‘s condition, and medical records;
- Review data from the Physical Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
- Review effective treatment strategies for areas of strength, endurance, and functional mobility with the Physical Therapist;
- Monitor and communicate patient response to physical therapy treatment;
- Assist the Physical Therapist with patient at discharge including patient and family instruction in safety, mobility and function;
- Attend meetings in regards to patient care such as care plan meetings , welcome meetings and daily huddles on an as needed basis;

- Record treatment, response, and progress in the electronic medical record. Documentation of patients care and services will adhere to accepted regulatory, corporate and professional guidelines;

- Educate the patient and/or caregivers to enhance functional independence;

MINIMUM QUALIFICATIONS:
- Physical Therapy Assistant degree from an accredited institution required.
- Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety.
- CPR and Pennsylvania license as a Physical Therapy Assistant required.
- Must also possess and independent supervision license.
- Valid Driver's license
