JOB SUMMARY:
The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services while operating in a resident centered environment. This position will also service as the Admissions Coordinator, playing a crucial role in assisting patients and their families during the admissions process. Their ability to effectively communicate information, handle phone inquiries, and coordinate with the admissions team contributes to providing a positive experience for patients seeking care.
TYPICAL JOB DUTIES:
- Completes an initial comprehensive psychosocial assessment of patients.
- Must be an advocate for the resident to resolve crises.
- Collaborates with other professionals to evaluate the physical, mental, and social aspects of the resident.
- Monitor, evaluate, and record residents’ progress according to measurable goals described in treatment and care plan.
- Modify treatment plans to comply with changes in residents' status.
- Identify environmental impediments to the residents’ progress through interviews and review of resident records.
- Counsel residents in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
- Educate residents and family members on the roles and levels of healthcare, community resources, entitlements, and advanced directives.
- Arranging for resources/funds to finance medications, durable medical equipment, and other needed services.
- Coordinates resident discharge and continuity of care planning.
- Answering phone calls and inquiries from potential patients and their families
- Providing information about the admissions process and requirements
- Assisting with the completion of necessary paperwork and documentation
- Coordinating with the admissions team to ensure a smooth transition for patients
- Maintaining accurate records of all admissions activities
MINIMUM QUALIFICATIONS:
- A bachelor’s degree in a Human Services field including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited institution.
- One year of supervised social work experience in a health care setting working directly with individuals.
- Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.

The LECOM Institute for Successful Living is currently accepting applications and resume submissions for a Home Health Physical Therapist Assistant to work with the LECOM Visting Nurse Association of Erie County.
Under the supervision of a Physical Therapist, the Physical Therapy Assistant administers medically prescribed physical therapy treatment for patients suffering from injuries and muscle, nerve, joint and bone disease, to restore function, relieve pain, and prevent disability by performing a numbered of required duties for the LECOM Institute for Successful Living.
Both Full Time and PRN scheduling options are available.
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Review physician orders, patient‘s condition, and medical records;
- Review data from the Physical Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
- Review effective treatment strategies for areas of strength, endurance, and functional mobility with the Physical Therapist;
- Monitor and communicate patient response to physical therapy treatment;
- Assist the Physical Therapist with patient at discharge including patient and family instruction in safety, mobility and function;
- Attend meetings in regards to patient care such as care plan meetings , welcome meetings and daily huddles on an as needed basis;
- Modify physical therapy treatment and plan of care according to patient’s ability and response as directed by the supervising Physical Therapist;
- Instruct patient and family in treatment procedures to be continued at home;
- Record treatment, response, and progress in the electronic medical record. Documentation of patients care and services will adhere to accepted regulatory, corporate and professional guidelines;
- Record daily treatment charges per corporate procedures’
- Adhere to all HIPPA procedures and guidelines in accordance with LECOM Health’s standard of excellence;
- Educate the patient and/or caregivers to enhance functional independence;
- Comply with federal, state and local legal and professional requirements;
- Maintain compliance with appropriate therapy minutes, therapy intensity, and productivity standards on daily basis;
- Work as scheduled, ready to devote full attention and energy to the important work of LECOM Health; and
- Accept other duties as needed/assigned for the Institution’s needs.
MINIMUM QUALIFICATIONS:
- Physical Therapy Assistant degree from an accredited institution required.
- Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety.
- CPR and Pennsylvania license as a Physical Therapy Assistant required.
- Must also possess and independent supervision license.
- Valid Driver's license

Job summary:
The main occupational tasks will involve creating images of the heart, cardiac chambers, valves, and vessels for the purpose of assessing heart function and diagnosing heart ailments.
Characteristic job tasks and responsibilities:
- The Tech will work closely with the physician/cardiologist, often performing the technical aspects of the examinations, evaluating the findings, and reporting them to the physician/cardiologists
- The ECHO Tech will perform ECHOs on both inpatients and outpatients. He/she will be responsible for interviewing the patients and reviewing their medical records so that he/she may adapt their sonographic techniques as necessity dictates.
- The Tech will provide patient care, such as preparing the patients for the sonographic imaging and providing them with instruction during the echocardiograph so that the images turn out properly.
- There will be clerical duties related to completion of the ECHO and submitting information to the physician/cardiologist.
- The tech must be familiar with preparation and administration of contrast such as Definity.
- Performs other duties as assigned or required.

Main Duties and Responsibilities:
1. Assist in patient care in the exam room as directed by physician
2. Perform clinical procedures under supervision of physician
3. Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
4. Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations
5. Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit. Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record. (pre-visit planning/chart prep)
6. Schedule tests, surgical procedures and coordinate referrals to provide continuous care for patients
7. Obtain all necessary authorizations as mandated by patient’s insurance
8. Track referrals for the practice to completion (report/result received and patient notified)
9. Perform lab draws, EKG’s and administer injections.
1. Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician
2. Keep exam rooms and medication storage room stocked with adequate medical supplies, maintain instruments, prepare sterilization as required
3. Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
4. Triage for urgency and process messages from patients and front office staff to physicians
5. Provide response to patient inquiry and concerns with attention to confidentiality
6. Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, cold sterilization fluid change, etc.)
7. Keep patients in waiting area informed of any unusual delays in physicians’ schedule

SUMMARY: Responsible for accurate and timely processing of the complete General Ledger Accounting function, including journal entries, account reconciliations, and all associated reporting and functional processes. Assists with other accounting duties as required or instructed.
ESSENTIAL SKILLS:
· Proficient in analyzing, maintaining, and processing financial data.
· Must be proficient with Microsoft Excel, with a minimum of an intermediate user level (example is that you must be able to make and use Pivot tables in Excel).

DUTIES AND RESPONSIBILITIES:
· Generates reports and analysis of hospital incomes, expenditures, and capital asset values with comparisons to previous periods.
· Assists CFO and Senior Staff Accountant with information gathering and compilation of data for regulatory reports, internal management reports, budgeting, and the year audited statements.
· Creates and maintains departmental reports and records and collects statistical data for departmental, administrative, and regulatory purposes.

· Prepares and processes monthly general and standard journal entries, monthly account reconciliations, and banking account reconciliations.
· Works with auditors and/or interested outside parties, to provide and explain information and reports that are requested throughout the year.
· Process and track intercompany transactions and reconcile balances with other entities.
· Complete daily cash deposits and reconciliations.
· Performs maintenance and filing of documents and reports (either in hard copy or electronic format).
· Assists in the care and maintenance of department equipment and supplies.
· Serve as a backup (via cross training) in other areas of accounting beyond the typical normal accounting duties of the position.
· Maintains established policies and procedures, objectives, quality assurance program and safety standards
· Attends meetings as required.
· Performs other related duties as assigned or requested.

Medical Associates of Erie is seeking a reliable, experienced medical assistant to guarantee that each patient visit and the logistical arrangements surrounding the visit are carried out with courtesy, caring and consistency, while assisting the physician in patient care and education.
Main Duties and Responsibilities:
- Assist in patient care in the exam room as directed by physician
- Perform clinical procedures under supervision of physician
- Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
- Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations
- Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit. Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record. (pre-visit planning/chart prep)

- Provide response to patient inquiry and concerns with attention to confidentiality
- Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, cold sterilization fluid change, etc.)
- Keep patients in waiting area informed of any unusual delays in physicians’ schedule

JOB SUMMARY:
The Wound Nurse is responsible for the coordination of wound care for the residents at the LECOM Institute for Successful Aging. These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through standard operating procedures of all wounds.
MINIMUM QUALIFICATIONS:
- Must be Licensed as a Practical Nurse or Registered Nurse in Pennsylvania.
- Wound Care Certification or Ostomy Certification is preferred but not necessary.
- 1-2 years related experience and/or training; or equivalent combination of education and experience.
- Must maintain current certifications in CPR and First Aid.
TYPICAL JOB DUTIES:
- Provide case management for consulting patients providing specific assessment according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patients’ wound care;
- Provides efficient and effective care with the guidance of the Physician during the wound clinic sessions, attends or follows up on all wound clinic appointments where scheduled. Able to manage a high number of patients at the same time with high energy level and organization;
- Collaborates with other health care providers, wound care Physicians, Nursing Leaders and floor staff, Home Health Agencies, Extended Care Facilities, and Wound Clinic Program Director and Clinical Coordinator, regarding resident needs;
- Will participate in the development and establishment of a goal oriented skin care protocol;
- Implementation of the skin care protocol and orders within 24 hours or next business day after admission;
- Order entry of initial LECOM Wound Clinic orders;
- Order entry as needed upon changes in skin care;
- Daily review of skin incidents and wound / ulcer report accuracy;
- Liaison to LECOM Health Wound Clinic for transfer assistance;
- Assist in resident / family / employee teaching and education as needed;
- Review , document and report significant changes as well as quarterly and annual care plans;
- If Patient has an active wound, will participate in multi-disciplinary team care plan meetings;
- If Patient has no active wounds, a wound nurse business card will be provided during the care plan meeting;
- Must provide daily skilled wound care on all patients, as applicable;
- Nursing staff will perform head to toe skin assessments upon admission or next business day after admission and communicate results to wound nurse;
- The wound nurse will assess either upon admission or on next business day after admission if there is any presence of a wound;
- The wound nurse will assess within 5 days of an admission if no wound is identified by the nursing staff in their head to toe assessment;
- and continue performing at least weekly skin assessments;
- Weekly wound care rounds, follow up and documentation.
