LECOM Health Corry Memorial Hospital is seeking a Nurse Aide for our Acute Care Unit.

  1. Assists patients with activities of daily living, provides basic nursing care to patients and assists in maintenance of a safe and clean environment under the direction and supervision of the Registered Nurse and the Licensed Practical Nurse.
  2. Assists in admission, transfer and discharge of patient.
  3. Performs basic patient care responsibilities appropriate to condition of the patient.
  4. Communicates to the RN/LPN any observed changes in patient status and responses.
  5. Assists with feeding patients and maintaining fluid balance with technique appropriate for patient’s conditions.
  6. Provides explanation to patient and family prior to administering care to patient; answers questions within scope of practice.
  7. Performs treatments and provides services within scope of practice, such as nutrition, mobility, hygiene, and the answering of call bells.

SUMMARY:

Self-starter; excellent time manager, and highly organized team focused individual with the ability to follow direction with minimal oversight, solve problems and communicate effectively with both internal and external customers and insurance payers within tight deadlines.

DUTIES AND RESPONSIBILITIES:

  • Prepare, review, and transmit facility and professional claims using billing software and clearinghouse, including electronic and paper claim processing within payer filing limits.
  • Ensure rejected claims are reviewed, corrected, and reprocessed timely.
  • Follow up on unpaid claims and contact payers on various issues within standard billing cycle timeframe.
  • Review payments for accuracy; be able to spot check for 100% reimbursement on charges, and excessively low payment.
  • Identify and bill secondary or tertiary insurance payers as appropriate.
  • Answers all patients, providers, or insurance telephone inquiries pertaining to assigned accounts and work to resolve them effectively.
  • Timely follow up on insurance claim denials, exceptions or exclusions.
  • Stays informed on local regulations and changes in billing requirements.
  • Make necessary arrangements for medical record requests, completion of additional information requests, appeals, etc. as requested by insurance companies.
  • Regularly meet with Patient Accounting Leader to discuss and resolve reimbursement issues or billing obstacles.
  • Regularly attend department huddles and meetings.
  • Perform additional duties as requested.

Minimum Education:

Minimum of associate degree required.

Minimum Experience:

Minimum of one to three years of experience in medical or hospital billing, hospital billing preferred.

Minimum Skills:

  • Demonstrate reliability and a sense of urgency in completing tasks quickly and in a timely manner.
  • Excellent customer service skills. Conceptual understanding of financial record keeping and medical billing procedures.
  • Competent use of computer systems, software (including Microsoft Office), and 10 key calculators.

SUMMARY: 

The Mental Health Professional I (MHP I) provides trauma-informed and resilience-oriented mental health therapy and supports to individuals, families, and/or groups. The MHP I conducts mental health intakes and assessments, diagnoses mental health disorders, and develops personalized treatment plans for individuals to address and manage their mental health concerns. This position requires the ability to provide intensive case coordination and therapeutic services to patients with an emotional disturbance or mental illness. The position requires the ability to design crisis safety plans and direct treatment through the LECOM Health network and other community partners. The MHP I works as an extension of the Outpatient Psychiatric treatment team with the core job duties being completed at one of the satellite offices of the clinic and/or other LECOM Health entities.

Main Duties and Responsibilities:

  • Mental Health Therapy
  • Provide individual, family, and/or group therapy as assigned by Supervisor.
  • Maintain responsibility for an assigned caseload of individuals addressing mental health concerns.
  • Provide both face-to-face and telehealth services to individuals.
  • Support the family's or other supports’ efforts to stabilize the identified individual receiving services.
  • Collaborate with other healthcare professionals to develop treatment plans for people with mental health conditions.
  • Assist individuals receiving care and their treatment teams with implementing, monitoring, and reevaluating crisis and safety plans.
  • Intake & Assessment
  • Complete biopsychosocial assessments of patients as part of an intake interview to identify possible factors contributing to individual’s overall physical and psychological health.
  • Assesses strengths and therapeutic needs of patient and family.
  • Assesses treatment adherence, tolerability, and clinical response using validated rating scales.
  • Help to triage patient need and coordinate assessing the need for sooner appointments with the treating provider and/or to be the liaison between the patient and the provider agency to relay information.
  • Assist patients with identifying safety plans and crisis plans.
  • Coordinate with the treatment team to monitor progress toward goals and outcomes.
  • Oversee the completion of all required paperwork and documentation in the patient’s chart within established departmental timeframes.
  • Case Management
  • Provide case management which would include, but not be limited to coordination of treatment with other service providers, assistance to families in locating needed resources, and patient advocacy.
  • Outreach and engagement of patients after visits.
  • Track referrals to completion and contact patients who have not yet followed up with recommended community resources.
  • Help patients to link to community resources such as: drug and alcohol services, BCM, transportation services, physical health resources, education, etc.
  • Coordinate with other service providers such as therapist and primary care.
  • Work collaboratively with the treatment team and serve as the liaison between patients and medical staff.
  • Develop and/or complete written patient documentation/letters as medically necessary and in coordination with approval from the Medical Director.
  • Documentation & Supervision
  • Complete timely clinical documentation in the health record regarding all contact with the individual receiving services.
  • Ensure all relevant and collateral record documents are uploaded for patient based upon dialogue with patient, family, and other collaborating providers.
  • Ensure all incident reports are completed timely and reviewed with Supervisor.
  • Participate in case review and interdisciplinary team meetings with psychiatrist.
  • Receive clinical supervision and maintain supervision log.

Other Duties:

  • Implement all relevant policies and procedures.
  • Attend all meetings, clinical supervisions, and training sessions across any of the Licensed Outpatient Clinic settings.
  • Understand and adhere to the professional policies and standards in the Millcreek Community Hospital Employee Handbook.
  • Act as a Goodwill ambassador for LECOM Health by promoting awareness to the patient of all physicians and services LECOM Health provides.
  • Adhere to obligations under clinic’s Duty to Warn policy.
  • All other duties as assigned.

Skills/Knowledge:

  • MHP I qualifications meets one of the following:
  • Has a graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum.
  • Has an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. (AICE) or the National Association of Credential Evaluation Services (NACES).
  • The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.
  • Licensed as LCSW or LPC preferred
  • Possesses knowledge of Erie County mental health resources and mental health level of care determination.
  • Skilled in therapeutic interventions.
  • Must be able to obtain Act 33/34 and FBI clearances upon hire.
  • Must complete Mandated Reporter training within 30 days of hire.
  • Must complete crisis management and de-escalation training.
  • Exceptional customer service and phone etiquette.
  • Ability to maintain effective and organized systems.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

  • Frequent standing/walking/sitting
  • Occasionally lifting items of light to medium weight (up to 25lbs).

  1. Advertise and market vacant properties to find tenants as needed
  2. Work with Finance to collect rent and other property fees from tenants and individual owners
  3. Reporting the property’s, occupancy and expiring leases to management
  4. Meeting potential tenants, showing them the property and assessing their applications in accordance with anti-discrimination laws
  5. Inspecting properties and arranging for repairs and new materials as required
  6. Arranging contracts for maintenance, trash removal, landscaping, security and other ongoing services and managing disputes with these service providers where appropriate
  7. Working with utility companies to properly maintain service during tenant move in/out
  8. Investigating and resolving property complaints and rental violations
  9. Performs clerical duties for the hospital and MCH Corp.
  10. Maintains established departmental policies and procedures, objectives, quality assurance programs, and safety standards.
  11. Answers phones, processes mail, photocopies, and prepares departmental correspondence.
  12. Maintains files and records pertaining to facilities management. Documents include, but are not limited to purchasing, invoices, drawing, permits, certificates, correspondence, and Life Safety documents needed to maintain compliance with Federal, State and Local codes.
  13. Creates and modifies word processing, spreadsheet, drawing, and pdf documents.
  14. Receives and processes departmental work orders. Schedules vendors and other Plant Operations Staff for preventative maintenance processes, repairs, and other projects.
  15. Assists in maintaining a computerized work order system.
  16. Contacts vendors, gathers quotes and places orders for maintenance services and supplies.
  17. Produces and maintains property leases and related records and correspondence.
  18. Assists with off-site property inspections to document the condition of rental properties, and makes recommendations for property improvement.
  19. Maintains professional communication with tenants, co-workers, vendors, and others.
  20. Documents and responds to complaints from neighbors and others regarding off-site properties.
  21. Communicates with tenants regarding lease requirements, late payments, and security deposit issues.
  22. Researches and documents local rental rates to establish MCH Corp lease rates.
  23. Performs other related duties as assigned or requested.

SUMMARY: Provides assistance in meeting the daily living needs of patients within the therapeutic setting. Directs patient activities designed to improve biopsychosocial functioning. Responsible for the implementing daily activities and for the supervision of patients.

DUTIES AND RESPONSIBILITIES:

  • Participates as a member of a multidisciplinary treatment team and provides feedback to Unit Leader and Case Manager.
  • Facilitates daily routine on the unit. These duties include:
    • Ensuring firm adherence to unit schedule
    • Passing meals
    • Providing positive support during academic instruction
    • Following a psycho-education & therapeutic curriculum designed by treatment team to lead & facilitate groups
    • Leading and participating in special activities on the unit
    • Occasionally preparing and leading groups
    • Ongoing monitoring of safety
  • Provides least restrictive intervention to ensure safe and supportive de-escalation of patients experiencing distress
  • Utilizes physical intervention in accordance with training when required to maintain physical safety of patients
  • Adheres to a Trauma Informed Approach
  • Maintains a safe and therapeutic environment and the supervision of the patients.
  • Completes timely and quality documentation, including shift note and

QUALIFICATIONS:

  • Education Requirements: High School Diploma or equivalent required B.A./B.S. in psychology or related field preferred
  • Minimum Experience: A minimum of one year of experience in behavioral healthcare setting preferred. 
  • Experience in residential or partial setting strongly preferred.
  • English Language Skills: Must be able to read, write and speak the English language in an understandable manner.
  • Physical Demands: Must be in good general health and demonstrate emotional stability.
  • Must be able to lift up to and in excess of 25 pounds throughout the workday. Sit, stand, bend, lift and move intermittently during working hours.

REQUIRED CERTIFICATIONS:

  1. CPR Certification
  2. Therapeutic Holds Training (*Both to be obtained within 60 days of hire.)
  3. Working Environment: Works in well lighted/ventilated areas. Subject to hostile and emotionally upset residents, family members, etc.

The LECOM Institute for Successful Aging is now accepting applicants for its Certified Nurse Aide course.

 

Applicants selected for the LECOM CNA course will receive their training at no cost, as well as books for the class. LECOM will also cover the cost for the licensing exam (terms apply).

Applicants will be offered a Full Time CNA position within one or more of our skilled nursing facilities, contingent upon successful completion of class and the final exam.

About the Class
The LECOM CNA Course is a 4 week course, with a total of 120 hours of classroom and clinical training. The first two weeks of the course will be classroom hours hosted at the Regency at South Shore, located at 322 Washington Place, Erie PA 16505, from 8:00 AM – 2:45 PM. Clinical hours will be held during the second two weeks of the course at the LECOM Senior Living Center, located at 5535 Peach St, Erie PA 16509, from 7:00 AM- 1:30 PM. Candidates who can work Full Time 2nd shift, 3rd shift, or 12 hour overnight shift upon graduation of the course will be given preference for admittance. Attendance is during the class is mandatory.

Qualifications

  • High School Diploma or GED equivalent, strongly preferred
  • Applicants must complete a pre-employment physical and criminal background check prior to beginning the class
  • Must have reliable transportation to and from the class location(s)

JOB SUMMARY:

The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services while operating in a resident centered environment.

TYPICAL JOB DUTIES:

  • Completes an initial comprehensive psychosocial assessment of patients.
  • Must be an advocate for the resident to resolve crises.
  • Collaborates with other professionals to evaluate the physical, mental, and social aspects of the resident.
  • Monitor, evaluate, and record residents’ progress according to measurable goals described in treatment and care plan.
  • Modify treatment plans to comply with changes in residents’ status.
  • Identify environmental impediments to the residents’ progress through interviews and review of resident records.
  • Counsel residents in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
  • Educate residents and family members on the roles and levels of healthcare, community resources, entitlements, and advanced directives.
  • Arranging for resources/funds to finance medications, durable medical equipment, and other needed services.
  • Coordinates resident discharge and continuity of care planning.

MINIMUM QUALIFICATIONS:

  • A bachelor’s degree in a Human Services field including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited institution.
  • One year of supervised social work experience in a health care setting working directly with individuals.
  • Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.
