About the LECOM Institute for Successful Living

LECOM Health offers a full continuum of care from Independent Living and Personal Care apartments to Skilled Nursing Care and Outpatient services including In-Home Care, Home Health, and Hospice under the direction of the LECOM Institute for Successful Living.

The Assistant Director of Nursing is a Registered Nurse (RN) who will be responsible for the day to day coordination and oversees all aspects of the Nursing Department as assigned by the Director of Nursing. The ADON is responsible for maintaining a high standard of patient centered care and is expected to plan and provide managerial and clinical support to patients and staff.

*This position is located at 4114 Schaper Ave, Erie PA.*

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Manages employees and participates in selection, orientation, counseling, evaluation, and scheduling of staff.
  • Provides and facilities resident centered care.
  • Performs routine skin evaluations and ensures the treatments are ordered and applied appropriately.
  • Serves as a resource person to residents, families, physicians and staff in interpreting department policy.
  • Assists DON in aspects of marketing including, but not limited to, developing positive relationships with the community, residents, and families. Providing tours and attending marketing events will also be required.
  • Assists in the negotiation of approval for resident stay and rates with payers.
  • Serves as an educator for residents, families, referral sources, payers, and case managers.
  • Participate in precertification, preadmission screenings, outcome data, and review of records.
  • Assists in the coordination of admissions and discharges.
  • Conducts resident interviews for social, personal, and medical history.
  • Assists residents with the activities of daily living including, but not limited to; bathing, dressing, dining, and transportation.
  • Assists in the care planning process including implementation and accurate and timely documentation.
  • Will perform as the Acting or Interim D.O.N. in the absence of the D.O.N.
  • Must be flexible to accept other duties as needed or assigned by the resident or institution.
  • Performs other duties as assigned by supervisor or management.

QUALIFICATIONS:

  • Must hold a valid PA RN License
  • Previous supervisor experience preferred.

  • Educate residents and their families on respiratory care techniques and self-management.

  • Document and maintain accurate records of respiratory assessments, treatments, and interventions.
  • Participate in multidisciplinary team meetings to discuss and coordinate resident care.

Qualifications:

  • Current Pennsylvania state licensure as a Respiratory Therapist.
  • Graduation from an accredited Respiratory Therapy program.
  • BLS and ACLS certification.
  • Previous experience in a skilled nursing facility or long-term care setting is preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative healthcare team.

The primary purpose of the LNAC is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Pennsylvania and the policies and goals of our facility. We strive to provide the best clinical outcomes for our residents, and the LNAC is a valuable member of the residents' Care Plan Team. Time management and attention to detail are crucially important to this role.

Job Responsibilities:

  • Conduct or coordinate the interviewing of each resident for the resident's assessment
  • Evaluate each resident's condition and pertinent medical data to determine the need for special assessment activities or need to amend the admission assessment
  • Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment function and that they are aware of the penalties, including civil money penalties, for false certifications
  • Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition
  • Coordinate the review and revision to the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status
  • Sign and date the assessment instrument to certify its completion
  • Responsible for transmission of resident information to the state per requirements

Education and Qualifications:

  • Graduate of an accredited Practical Nursing program required
  • Licensure and registration by the PA Board of Nursing Examiners as a Licensed Practical Nurse required
  • Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred.
  • Previous LNAC experience preferred.

SUMMARY: Assist the Psychiatric Resident and Attending Physician in the Behavioral Health area in the Emergency Department. Duties to include data collection and data entry into Electronic Medical Record; compiling the admission packet, coordination of admission/discharge information with the medical area in the Emergency Department; inform the Charge Nurse on Unit 4/5 of admission, confirming bed availability. This position will also assume responsibilities on Unit 2/4/5 when patients are not present in the Behavioral Health area in the Emergency Department.

DUTIES AND RESPONSIBILITIES:

1. Participates as a member of a multidisciplinary Emergency Department and Behavioral Health teams, attending to the responsibilities and needs of assigned patients in coordination with the Psychiatric Resident and Attending.

2. Completes initial psychosocial assessment with patients and their representatives presenting to Emergency Department seeking possible inpatient psychiatric hospitalization.

3. Participates in monitoring of a safe and therapeutic environment.

4. Completes safety checks of patients and patient belongings for contraband, to include wanding with metal detector. Following safety check, assists patients with changing into hospital gowns/clothing.

5. Documents patient behaviors and deviations in electronic medical record.

6. Inventories and documents patient belongings, placing home medications and valuables in appropriate hospital safes.

7. Completes insurance pre-authorizations and pre-certifications for patients being admitted for inpatient psychiatric treatment.

8. Assists with patient care, safety rounds, and other duties on Unit 4 and Unit 5 as requested by Charge RN, during times when no patients are present in Behavioral Health area of Emergency Department.

9. Demonstrates professional behavior and interactions at all times with patients, family members, other professionals and staff.

10. Participates in staff training programs designed to improve professional knowledge and skills that will enhance clinical expertise and patient care.

11. Executes other duties as assigned by the Director of Behavioral Health or Behavioral Health Unit Leader.

QUALIFICATIONS:

Education Requirements: High school diploma or equivalent

Minimum Experience: A minimum of one year of experience in behavioral healthcare setting.

English Language Skills: Must be able to read, write and speak the English language in an understandable manner.

Physical Demands: Must be in good general health and demonstrate emotional stability. Must be able to lift up to and in excess of 25 pounds throughout the workday. Sit, stand, bend, lift and move intermittently during working hours.

Job Duties:

  • Directs activities of patient accounting, coding and utilization review personnel to ensure efficient and effective generation of revenue and billing to maintain a stable cash flow.

SUMMARY: 

The Mental Health Professional I (MHP I) provides trauma-informed and resilience-oriented mental health therapy and supports to individuals, families, and/or groups. The MHP I conducts mental health intakes and assessments, diagnoses mental health disorders, and develops personalized treatment plans for individuals to address and manage their mental health concerns. This position requires the ability to provide intensive case coordination and therapeutic services to patients with an emotional disturbance or mental illness. The position requires the ability to design crisis safety plans and direct treatment through the LECOM Health network and other community partners. The MHP I works as an extension of the Outpatient Psychiatric treatment team with the core job duties being completed at one of the satellite offices of the clinic and/or other LECOM Health entities.

Main Duties and Responsibilities:

  • Mental Health Therapy
  • Provide individual, family, and/or group therapy as assigned by Supervisor.
  • Maintain responsibility for an assigned caseload of individuals addressing mental health concerns.
  • Provide both face-to-face and telehealth services to individuals.
  • Support the family's or other supports’ efforts to stabilize the identified individual receiving services.
  • Collaborate with other healthcare professionals to develop treatment plans for people with mental health conditions.
  • Assist individuals receiving care and their treatment teams with implementing, monitoring, and reevaluating crisis and safety plans.
  • Intake & Assessment
  • Complete biopsychosocial assessments of patients as part of an intake interview to identify possible factors contributing to individual’s overall physical and psychological health.
  • Assesses strengths and therapeutic needs of patient and family.
  • Assesses treatment adherence, tolerability, and clinical response using validated rating scales.
  • Help to triage patient need and coordinate assessing the need for sooner appointments with the treating provider and/or to be the liaison between the patient and the provider agency to relay information.
  • Assist patients with identifying safety plans and crisis plans.
  • Coordinate with the treatment team to monitor progress toward goals and outcomes.
  • Oversee the completion of all required paperwork and documentation in the patient’s chart within established departmental timeframes.
  • Case Management
  • Provide case management which would include, but not be limited to coordination of treatment with other service providers, assistance to families in locating needed resources, and patient advocacy.
  • Outreach and engagement of patients after visits.
  • Track referrals to completion and contact patients who have not yet followed up with recommended community resources.
  • Help patients to link to community resources such as: drug and alcohol services, BCM, transportation services, physical health resources, education, etc.
  • Coordinate with other service providers such as therapist and primary care.
  • Work collaboratively with the treatment team and serve as the liaison between patients and medical staff.
  • Develop and/or complete written patient documentation/letters as medically necessary and in coordination with approval from the Medical Director.
  • Documentation & Supervision
  • Complete timely clinical documentation in the health record regarding all contact with the individual receiving services.
  • Ensure all relevant and collateral record documents are uploaded for patient based upon dialogue with patient, family, and other collaborating providers.
  • Ensure all incident reports are completed timely and reviewed with Supervisor.
  • Participate in case review and interdisciplinary team meetings with psychiatrist.
  • Receive clinical supervision and maintain supervision log.

Other Duties:

  • Implement all relevant policies and procedures.
  • Attend all meetings, clinical supervisions, and training sessions across any of the Licensed Outpatient Clinic settings.
  • Understand and adhere to the professional policies and standards in the Millcreek Community Hospital Employee Handbook.
  • Act as a Goodwill ambassador for LECOM Health by promoting awareness to the patient of all physicians and services LECOM Health provides.
  • Adhere to obligations under clinic’s Duty to Warn policy.
  • All other duties as assigned.

Skills/Knowledge:

  • MHP I qualifications meets one of the following:
  • Has a graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum.
  • Has an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. (AICE) or the National Association of Credential Evaluation Services (NACES).
  • The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.
  • Licensed as LCSW or LPC preferred
  • Possesses knowledge of Erie County mental health resources and mental health level of care determination.
  • Skilled in therapeutic interventions.
  • Must be able to obtain Act 33/34 and FBI clearances upon hire.
  • Must complete Mandated Reporter training within 30 days of hire.
  • Must complete crisis management and de-escalation training.
  • Exceptional customer service and phone etiquette.
  • Ability to maintain effective and organized systems.
