This is the second level Plant Operations Specialist position. Employees at this level are expected to skilled in electrical, plumbing, HVAC, and carpentry trades, and be capable of performing repairs, improvements and preventative maintenance processes independently.

Duties and Responsibilities

1. Perform all duties of the Plant Operations Specialist I as well as the duties listed below.

2. Conducts troubleshooting techniques to determine the cause of equipment and system failures.

3. Monitors, repairs, installs, and adjusts all types of specialized hospital equipment and systems, such as oxygen systems, vacuum systems, medical gases and related systems, projectors, fire life safety systems, lights and other types of medical systems and equipment gases and related systems, projectors, fire life safety systems, lights and other types of medical systems and equipment.

  • Services air conditioning, refrigeration, dishwashers, air compressors, fans, pumps, motors, garbage disposals, heating and ventilation systems and devices, boilers, and associated equipment.

5. Performs preventive maintenance & make repairs to heating, ventilating and air handling equipment according to instructions

6. Perform carpentry work including framing walls, hanging and finishing drywall, leveling floors and installing floor tile, installing suspended ceilings, repairing and adjusting doors so that they operate and latch properly.

7. Troubleshooting electrical circuits, replacing and installing electrical devices, and diagnosing and repairing electrical equipment.

8. Performing plumbing repairs and installations.

9. Must be able to read and understand manufacturer’s publications and construction drawings in performance of duties.

10. Perform Fire Drills and other emergency management processes in accordance with hospital policies.

11. Conduct snow removal activities using a shovel, snow blower, and plow truck.

12. Perform and document preventative maintenance processes

13. Complete maintenance work orders in a timely manner and document the work properly.

14. Maintain a clean professional appearance.

15. Operate tools and equipment in a safe manner, including but not limited to Powered and non-powered hand tools, gas powered equipment like snow blowers, lawn mowers and string trimmers, air compressors, drain augers, trucks, vans, ladders, scaffolds, etc. …

16. Performs hospital rounds according to schedule and policy. Correct unsafe conditions in work area and report unsafe conditions to supervisor.

17. DON and DOFF appropriate PPE as advised to protect the employee from potentially harmful conditions.

18. Keeps facilities clean in appearance.

19. Performs other related duties as required

Other Duties as Assigned

Other duties can include:

1. Working at other LECOM facilities, including residential properties.

2. Filling in other roles when the hospital has urgent needs due to absences, disaster response, or safety issues that might arise.

3. Working other shifts temporarily to complete tasks that cannot be done during typical work hours.

SUMMARY:

The Mental Health Professional II (MHP II) conducts mental health intakes and biopsychosocial assessments, provides mental health therapy, and supports intensive case management needs for adults who have serious mental illness. The position requires the ability to design and direct the implementation of treatment plans and collaborate with treatment team members through the LECOM Health network.

Responsibilities include assessing the strengths and therapeutic needs of patients, formulating clinical impressions based on biopsychosocial assessments, assigning diagnoses, collaborating with patients to develop clinically sound treatment plans, providing culturally competent and patient-centered outpatient therapy, and ensuring flexible and accommodating scheduling. The MHP II must be comfortable with and capable of engaging with patients both face-to-face and via telehealth.

Main Duties and Responsibilities:

Customer Service

  • Assist potential patients with their questions and concerns.
  • Respond to patient inquiries in a professional and respectful manner.
  • Establish a collaborative relationship with team members, patients/families, and colleagues.
  • Respect patient/family preferences and choices for intervention and treatment.
  • Check voicemail and email throughout the day and respond to patient, colleague, and stakeholder messages within 24 business hours.
  • Adhere to HIPAA, licensure, accreditation, and confidentiality requirements of our patients.
  • Help connect patients and families with interpreters as needed.
  • Be a goodwill ambassador for LECOM Health by promoting patient awareness of all LECOM Health services and supporting Health system navigation needs.

 

Intake & Assessment

  • Complete biopsychosocial intake assessments with patients to identify factors impacting overall wellness and psychological health.
  • Assess strengths and therapeutic needs of patients.
  • Assist patients with treatment goal identification.
  • Assess treatment adherence, tolerability, and clinical response using validated rating scales.
  • Help to triage patient needs and adjust appointment intervals for therapy and/or the treating psychiatrist as necessary.
  • Coordinate with the treatment team to track treatment and outcomes.
  • Oversee the completion of required paperwork and documentation in the patient’s chart within established departmental timeframes.
  • Review all referrals and triages for clinical appropriateness of outpatient therapy services.

Mental Health Therapy

  • Engage in collaborative outpatient therapy services with patients.
  • Promote stabilization and progress toward identified treatment goals.
  • Formulate appropriate treatment, safety, crisis, and discharge plans.
  • Regularly monitor patient progress and reevaluate patient needs.
  • Help with system navigation to other service lines or levels of care as necessary.
  • Participate in scheduled individual and group supervision.
  • Attend case consultation meetings as scheduled.
  • Complete all documentation within the established timeframes.
  • Schedule appointments flexibly and in alignment with clinical need.

 

Case Management

  • Provide case management and crisis support services which would include, but not be limited to, coordination of treatment with other service providers, assistance to families in locating needed resources, and patient advocacy.
  • Outreach to and engagement of patients between visits as clinically necessary.
  • Track referrals to completion and contact patients who have not yet followed up with recommended community resources.
  • Help patients gain access to community resources such as: drug and alcohol services, BCM services, transportation services, physical health resources, etc.
  • Help in a crisis or emergency and link patients to higher levels of care such as inpatient.
  • Coordinate with other service providers such as case managers and primary care.
  • Work collaboratively with the treatment team and serve as the liaison between patients and medical staff.
  • Support psychotropic medication management as prescribed by medical providers, focusing on adherence and effectiveness of treatment.
  • Make outreach calls and consult in between visits with the psychiatrist to evaluate stability.
  • Help to coordinate long acting injectable and Clozaril patient care.

 

Documentation & Supervision

  • Ensure proper documentation in patient records based upon dialogue with patient, family and other providers.
  • Review progress notes and treatment plans in patient files written by other members of the treatment team.
  • Participate in psychiatric case review meetings.
  • Participate in clinical supervision and maintain supervision log.
  • Maintain professional license.

 

Other Duties:

  • Be familiar with and implement all relevant office policies and procedures
  • Attend all office meetings, clinical supervisions, and training sessions
  • Provide support to program interns via appointment shadowing and dialoguing to aid in the furtherance of their study and skill practice.
  • Understand and adhere to the professional policies and standards in the Millcreek Community Hospital Employee Handbook
  • Adhere to obligations under clinic’s mandated reporter policy.
  • Adhere to obligations under clinic’s duty to warn policy.
  • All other duties as assigned.

Skills/Knowledge:

  • Graduate Degree in mental health field required.
  • Current PA LPC or LCSW Licensure (or license eligible) preferred.
  • Possesses knowledge of Erie County Mental Health Resources and mental health level of care determination.
  • Knowledgeable and Skilled in therapeutic interventions for adults.
  • Must be able to obtain Act 33/34 and FBI clearances upon hire.
  • Must complete Mandated Reporter training within 30 days of hire.
  • Exceptional customer service and phone etiquette.
  • Ability to maintain effective and organized documentation.

QUALIFICATIONS:

Minimum Education: Bachelor’s Degree in Computer Science, MIS, or a related degree, with relevant healthcare I.T. experience.

Minimum Experience: Three years recent experience in network and hardware support, or an associated degree in computer technology and five years experience in network and technical implementations and support.

Minimum Skills: Broad knowledge of systems administration and management, network operating systems, network hardware, software, telecommunications systems and services and backup procedures.

Experience with system planning, development and management of systems which include enterprise directories, email and Internet services. Ability to read, analyze and interpret technical documentation. Ability to carry out instructions furnished in written, oral or diagram form.

Knowledgeable support staff are available to assist you with questions regarding scheduling, medication refills, etc. The support staff functions as a liaison between the patients and the psychiatrists. As an affiliate of the Lake Erie College of Osteopathic Medicine (LECOM), our practice is engaged in the education of future behavioral health providers. Fellows, residents, and students work closely with care team members to gain specialized knowledge in the clinical practice setting.

This role is tasked with providing a full range of patient care responsibilities for a busy outpatient mental health center. The BH Clinical Assistant I guarantees that each patient visit and the logistical arrangements surrounding the visit are carried out with courtesy, professionalism, and compassion, while assisting the physician in patient care and education. The Clinical Assistant may work with more than one specific physician based on clinic needs each day as well as willingness to work at both LIBH settings. Flexibility in scheduling is preferred as we staff our offices M-Th 8-6:30pm, Friday 8-5. Experience working in behavioral health and/or outpatient office setting is highly preferred.

Main Duties and Responsibilities:

Direct Patient Care

· Greet patients warmly, escort patients to exam rooms.

· Measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record.

· Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed documents.

· Assist in patient care in the exam room as directed by physician.

· Perform clinical procedures under supervision of physician.

· Perform screenings per provider guidelines.

· Perform lab draws and administer injections.

· Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician.

· Keep patients in waiting area informed of any unusual delays in physician’s schedule.

Administrative Patient Care

· Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit.

· Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record.

· Schedule appointments, tests, and procedures to provide continuous care for patients.

· Support the obtaining of all necessary authorizations as mandated by patient’s insurance.

· Process prescription refills, and direct/instruct patients and/or family members regarding prescription and treatment instructions according to provider guidelines and orders.

· Aid in coordination and administration of long acting injectables.

· Assist with coordination of Clozaril patient care when requested by Office Lead and/or Clinic Director.

· Support the tracking of patient appointments, tests, and procedures through completion and maintain documentation of efforts in EHR.

· Keep exam rooms and medication storage areas stocked with adequate medical supplies.

· Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls.

· Triage for urgency and process messages from patients and front office staff to physicians.

· Provide response to patient inquiry and concerns with attention to confidentiality.

· Document all patient interactions and contact in the patient chart.

· Ensure daily documentation meets standards, regulations, policies, and practices.

· Keep appropriate records within patient’s EHR chart.

· Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, etc.)

· All other duties as assigned

Other Duties:

1. Assist in answering phone, scheduling appointments, and calling no-shows.

2. Be familiar with and implement all office policies and procedures.

3. Clean equipment, treatment spaces, etc. between visits to maintain infection control as needed.

4. Attend all office meetings and training sessions on time.

5. Perform a variety of routine clerical duties including but not limited to filing, copying, faxing, sorting mail and posting, etc.

6. Understand and adhere to the professional policies and standards in the Employee Handbook.

7. Act as the Goodwill ambassador for the clinic, the hospital and the LECOM Health System by promoting awareness to the patient of all physicians and services LECOM Health provides.

8. Provide clinical coverage at other locations when needed.

· Ability to perform phlebotomy and administer injections

· High School Diploma/Medical Assistant Training preferred

· Medical Assistant certification helpful

· Knowledge of health care field and medical terminology

· Maintain patient confidentiality and adhere to HIPAA regulations

· Exceptional grammar, spelling and punctuation

· Maintain CPR/First Aid certification

· Must be able to obtain Act 33/44 and FBI clearances upon hire.

· Must complete Mandated Reporter training within 30 days of hire.

· Must complete Crisis Management Training upon hire.

DUTIES AND RESPONSIBILITIES:

1. Assembles prepared food on trays for general and therapeutic diets.

2. Maintains tray line in a clean, neat and orderly manner.

3. Delivers food trucks to patient floors as specified. Returns them to the dish room at designated.

4. Tends and loads dish machine in patient/employee dish room.

5. Washes certain items by hand such as pot, pans and utensils.

6. Disposes of trash and garbage as scheduled. Cleans and sanitizes trash

containers as they are emptied.

7. Cleans and sanitizes all food and supply carts in the department at the end of each shift.

8. Is responsible for cleaning all work tables and surrounding work areas in the dish room and pot washing area at the end of each meal period. Sanitizes after cleaning.

9. Sweeps and mops floors throughout the department at designated times.

10. Maintains clean and sanitary dining areas prior to and following each meal period.

11. Assists in preparing such items as salads, dessert or nourishments.

12. Assists in the storage of food and supplies following deliveries using the first-in, first-out method.

13. Prepares and delivers late trays as instructed.

14. Performs heavy cleaning following the cleaning schedule.

15. Assists in the set-up, service and clean-up of special meetings and functions.

16. Must maintain a current record of attendance to the required inservices as determined by the institution on an annual basis.
