Summary:
Duties to include charting, MDS responsibility, resident assessments, Diet recommendation and nutritional consults. Will assist to provide RD / DTR coverage throughout the LECOM continuum of care as well as provide services to senior adults throughout the community. Will take direction from lead building RD and Directory of Culinary Services for Lecom Senior Living communities.

Job Summary
We are looking for a reliable Full Time Transportation Assistant to act as a communication point for emergency and non-emergency calls. You will receive requests, transmit messages and track vehicles.
The ideal candidate must be primarily an excellent communicator and able to remain calm and composed, especially in emergency situations. You must be able to multi-task as well as take the appropriate action with little supervision.
The goal is to enable different parties to communicate well by ensuring the accurate and timely transmission of information.
Responsibilities
- Receive emergency and non-emergency calls and record significant information
- Address problems and requests by transmitting information or providing solutions
- Receive and dispatch orders for transport of patients, residents, or deliveries
- Prioritize calls according to urgency and importance
- Use radio, phone or computer to send crew, vehicles or other units to appropriate locations
- Monitor the route and status of field units to coordinate and prioritize their schedule
- Provide field units with information about orders, traffic, obstacles and requirements
- Enter data in computer system and maintain logs and records of calls, activities and other information
Requirements and skills
- Proven experience as dispatcher or relevant position
- Tech-savvy with knowledge of relevant methods
- Fast typing with experience in data entry
- Knowledge of procedures and guideline for emergency situations
- Proficient in English (oral and written)
- Outstanding organizational and multitasking abilities

The Program Coordinator / GME Program Coordinator is responsible for developing, overseeing, and improving The Graduate Medical Education Residency Program under the direction of the Program Director, implementing changes based on the current accreditation requirements, and preparing for accreditation site visits and review by the ACGME Review Committees.
The ideal Candidate will possess prior Healthcare or Project Management experiences who have the ability to organize materials, scrutinize and interpret regulations and guidelines to create and improve programs and directives while multi-tasking multiple projects at the same time.
- Maintain and appropriately communicate the Program Director’s schedule
- Schedule and coordinate meetings and conference calls
- Coordinate the Program Director correspondence
- Provide administrative support to Program Directors, GME Administration, residents and fellows as needed across all programs
- Assist with documentation for resident and fellow training, including but not limited to scheduling meetings, problem solving, and maintaining program and institution files
- Assist in ACGME credentialing process for residency and fellowship, i.e. gathering information for Program Information Forms
- Prepare and assist with all aspects of accreditation requirements, site visits and inspections

- Schedule Resident/Faculty Workshops, meetings, facilities and catering
- Serve on the Orientation Planning Committee

- Ensure all phone calls are answered and/or redirected accordingly in a professional manner
- Open and distribute pertinent mail
- Type memos, correspondence, reports and other documents
- Attend and record minutes at various Committee meetings, and compose agendas as needed, including GME, GMEC, and patient safety
- Maintain program organizational files
- Coordinate all necessary certifications for incoming and existing trainees, including but not limited to BLS, PALS and ACLS
- Complete requests for credentials and verification of training forms
- Assist with licensure
- Facilitate Resident/Fellow membership in appropriate organizations as well as subscriptions/generate user accounts as needed
- Provide continuous innovation as relates to job duties and department
- Respond to all communications in a timely manner

SUMMARY: Responsible for duties and processes related to medical record retrieval, assembly, maintenance and processing. Responsible for scanning, analyzing and release of information in accordance with organizational timelines/policies and state/federal regulations. The HIM Technician safeguards and protects patient’s right to privacy and maintains confidentiality.
DUTIES AND RESPONSIBILITIES:
1. Assembles charts of all discharged inpatient and outpatient in the prescribed fashion, making sure that each page carries the patient’s name and medical record number. Following the established order of assemble and filing protocol as referenced by facility policy and procedure; completes scanning of all records. After scanning, verify all documents are completed and proper form ID’s are applied.
2. Ensures records are complete and properly stored and/or organized before filing or archiving.
3. When directed, prepares records for purging and/or shredding and may participate in medical record destruction with direct supervision.
4. Examines medical records for current and discharged patients for completeness. Medical records are analyzed for deficiencies as defined by state and federal medical record requirements, and hospital policies. Deficiencies are tagged for completion and entered into the EMR.
5. Responsible for verifying the removal of deficiencies once completed by provider.
6. Tracks unaddressed chart delinquencies, sending notifications to applicable parties at designated points in time as specified by departmental policy.
7. Processes subpoenas, court orders, legal requests and other non-routine requests for information in addition to routine requests.
8. Answers telephones promptly and courteously. Routes calls, takes messages, and provides routine information to customers as appropriate.
9. Communicates cooperatively and constructively with physicians, patients, guests, and members of the healthcare team to ensure accurate and timely completion of processes and continuity of services.
10. Responsible for safeguarding patient records and protecting patients’ right to privacy by ensuring compliance with HIPAA standards and maintaining confidentiality in all phases of work.
11. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
12. Performs other related duties as assigned or requested.
QUALIFICATIONS:
Minimum Education: High school or equivalent
Minimum Experience:
· 1-3 years of office experience required
· Knowledge of the medical record format, physiology and disease process, and understanding of medical terminology preferred. If no experience, strong interest in learning.

Responsibilities specific to Utilization Review include performing admission and concurrent review, and at times retro-review of all in-patients conforming to Medicare, Medicaid, and/or other Third Party Payer requirements. Responsibilities specific to Quality Assurance include performing surveillance and data collection as directed for trend recognition and development of effective actions/plans.

Maintains established departmental policies and procedures, objectives, quality, assurance program, safety, environmental and infection control standards
Issues, stores and inventories hospital linen for all departments of the Hospital
Sorts, inspects, counts and records laundered items received from Linen Services. Fills departmental requisitions by counting out, assembling into packs and placing on carts for delivery to designated departments.
Inventories linen items periodically to determine replacement needs in order to maintain adequate supply
Performs a variety of general cleaning tasks including cleaning offices, conference rooms and other assigned areas of the hospital as directed by Environmental Services Supervisor.
Must be able capable of lifting 50 pounds and be able to load linen carts and transport to various locations of the hospital.
Must have some knowledge of computers and working with Excel spreadsheets.
Must take care of equipment and supplies. Maintain care of handling lost and found articles.
Must be able to organize and plan work in order to get it completed in an eight hour day.

SUMMARY: Provides professional nursing care under supervision for assigned patients in in the Detox Unit, according to established standards and practices.
QUALIFICATIONS:
Minimum Education: Post High School Vocational/Specialized Training
Minimum Experience: Sufficient previous clinical experience in area of assignment
Required Course(s)/Training: Program for license and CPR certification
Req. Certification/Registration: Current LPN license and CPR certification
DUTIES AND RESPONSIBILITIES:
1. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
2. Cooperates with other personnel to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection control policies.
3. Cleans and prepares rooms between procedures and after terminal cleaning. Monitors and restocks supplies.
4. Monitors medical condition of patients and reports changes to appropriate personnel.
5. Demonstrates sensitivity to patient’s comfort and privacy and expresses interest in their progress.
6. Monitors emotional stability of patients and reports changes to physician or charge nurse.
7. Assists physicians with examinations, procedures and other processes related to direct patient care.
8. Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards.
9. Performs basic nursing procedures including thermal applications, non-sterile soaks, application of simple dressings, perennial care, and urine tests.
10. Prepares, hangs, monitors and discontinues I.V. fluids.
11. Admits patients and arranges emergency operations and other services.
12. Ensures an adequate stock of supplies and proper functioning of equipment.
13. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
14. Attends meeting as required.
15. Performs other related duties as assigned or requested. Interested.
