The Recreation Manager is responsible for the preparation and implementation of resident activities within the resident centered care environment. Will manage Certified Nursing Aides in Recreation Assistant positions on each neighborhood.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Knowledge, skills, attitude, and energy to consistently provide resident centered care.
  • Manage Activities Staff specifically Recreation Assistants for each neighborhood.
  • Manage Volunteer Staff.
  • Planning of daily, weekly, monthly and yearly activity calendars that meet the needs of residents and neighborhoods.
  • Planning of special events (dances, theme dinners, holidays, picnics, luncheons and outings).
  • Scheduling Entertainment including obtaining W-9s and invoicing.
  • Scheduling Clergy and Church visits to fit the religious needs of the population.
  • Assisting residents with Voting (registering residents to vote, obtaining absentee ballots, escorting them to the polls).
  • Complete admissions paperwork including but not limited to assessments and progress notes.
  • Responsible for completing MDS, attendance records and keeping care plans up to date.
  • Scheduling and running the Monthly Resident Council Meetings for each Neighborhood.
  • Managing the Pet Therapy program including scheduling and maintaining current pet records.
  • Ordering and distribution of appropriate magazines, newspapers and reading materials for each Neighborhood.
  • Manage the Local Library Program (tracking requests, books signed out)
  • Must perform as a Universal Worker being cross-trained in housekeeping, dietary and personal care functions.
  • Must be flexible to accept other duties needed or assigned by the resident or institution.
  • Performs other duties as assigned by supervisor or management.

The LECOM Institute for Successful Living, is looking for a caring, compassionate, and detail oriented Full Time Assessment Coordinator to join our growing team!

The primary purpose of the Assessment Coordinator is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Pennsylvania and the policies and goals of our facility. We strive to provide the best clinical outcomes for our residents, and the Assessment Coordinator is a valuable member of the residents' Care Plan Team. Time management and attention to detail are crucially important to this role.

Job Responsibilities:

  • Conduct or coordinate the interviewing of each resident for the resident's assessment
  • Evaluate each resident's condition and pertinent medical data to determine the need for special assessment activities or need to amend the admission assessment
  • Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment function and that they are aware of the penalties, including civil money penalties, for false certifications
  • Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition
  • Coordinate the review and revision to the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status
  • Sign and date the assessment instrument to certify its completion
  • Responsible for transmission of resident information to the state per requirements

Education and Qualifications:

  • Graduate of an accredited Nursing program is required
  • Licensure and registration by the PA Board of Nursing Examiners as a Registered or Practical Nurse is required
  • Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred.
  • Previous Assessment Coordinator experience highly preferred, but not required.

  • Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.
