Position Overview: The Social Worker / Parkside Coordinator will play a pivotal role in delivering comprehensive care coordination, case management, and community resource integration across multiple LECOM Institute for Successful Living facilities—including Regency at Southshore, Parkside at Glenwood, and Parkside Northeast.

Responsibilities

  • Conduct assessments and develop individualized care plans for residents.
  • Provide crisis intervention and support during challenging situations.
  • Collaborate with healthcare teams to ensure comprehensive care delivery.
  • Facilitate group sessions and individual counseling as needed.
  • Advocate for residents' needs within the facility and community resources.
  • Monitor progress and adjust care plans based on resident feedback.
  • Educate families about available resources and support systems.
  • Maintain accurate documentation of resident interactions and progress.

Qualifications

  • Master’s degree in Social Work (MSW) or equivalent experience required.
  • Valid state licensure as a Social Worker.
  • Strong knowledge of crisis intervention techniques and behavioral health practices.
  • Experience working with senior populations preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Familiarity with Medicare regulations is a plus.

ATHLETIC TRAINER

Position Description — NON‑EXEMPT

OVERALL OBJECTIVE OF JOB

The Athletic Trainer assists in the oversight and delivery of comprehensive health care services for athletes within the guidelines established by LECOM Health Orthopedics and Sports Medicine. This hybrid role includes sports medicine game coverage, athletic training room availability, and orthopedic sports medicine clinic patient care.

ESSENTIAL FUNCTIONS OF THE JOB

  • Distribute and collect required health documentation for athlete participation.
  • Monitor pre‑existing health conditions of involved athletes.
  • In the absence of a physician’s order, assist with decisions regarding athlete participation.
  • Assist sports medicine physicians with the care and evaluation of injuries for all sports sponsored by LECOM.
  • Provide game coverage for assigned sports.
  • Assist with recommendations regarding equipment and supplies.
  • Consult in the purchase and fitting of protective equipment and devices.
  • Support learning experiences for health professional trainees and, where appropriate, athletic training students.
  • Assist in formulating and monitoring treatment and rehabilitation plans.
  • Guide athletes regarding the need for physician referral.
  • Coordinate insurance information with the Business Office as related to injuries sustained during athletics.
  • Support the mission and vision of LECOM Health.
  • Perform other related duties as assigned.

OTHER DUTIES OF JOB

  1. Attend required meetings and trainings.
  2. Perform additional job‑related duties as needed.

SUPERVISION RECEIVED

Receives instruction and supervision from the Sports Medicine Physician and the Executive Director of MAE.

SUPERVISION GIVEN

Supervises assigned clinic staff.

WORKING CONDITIONS

  1. Works indoors in clinical and athletic environments with appropriate lighting, temperature, and ventilation.
  2. Exposure to normal levels of noise and potential for frequent interruptions.
  3. Exposure to dust/dirt in athletic settings.
  4. Interaction with injured, distressed, or emotionally upset individuals may occur.

PHYSICAL/MENTAL REQUIREMENTS

  • Ability to record, convey, and present information clearly.
  • Ability to alternate between extended standing, walking, and periods of sitting; occasional bending, lifting, and reaching required.
  • Dexterity required for coordination of hands, fingers, legs, and torso.
  • Must be able to lift/carry up to 10 pounds occasionally.
  • High attention to detail and ability to concentrate on complex tasks.
  • Must demonstrate emotional stability and sound judgment.

POSITION QUALIFICATIONS

Education & Training

  • Master’s Degree required.
  • Graduate of a CAATE‑approved program preferred.

Work Experience

  • Completion of a Master’s entry‑level CAATE‑approved training program preferred.

Certificates & Licenses

  • Certification in First Aid and CPR by the American Red Cross required.
  • Minimum one year of BOC certification preferred.

Knowledge, Skills & Abilities

  • Strong interpersonal and oral communication skills.
  • Ability to coach, mentor, and support athlete development.
  • Knowledge of health care principles for athletes and basic leadership practices.
  • Ability to work effectively with athletes, staff, supervisors, and the public.

The Chaplain/Bereavement Coordinator provides counseling or arranges counseling to meet the spiritual needs of the patients/family members/caregivers in a manner consistent with their beliefs and desires and in accordance with the patient’s plan of care.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Conduct a spiritual assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.
  • Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.
  • Provide spiritual support in accordance with on-going wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality.
  • Work in close collaboration with local clergy when desired by the patient.
  • Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
  • Evaluate the patients and family’s response to, and effectiveness of, the spiritual counseling.
  • Confirm, on a weekly basis, the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits.
  • Participate in IDG and QAPI meetings and activities.
  • Conduct religious services of prayer, worship and rituals for patients/families, as appropriate.
  • Working with other professionals in resolving spiritual/ethical issues.
  • Provide spiritual and bereavement support to staff as needed.
  • Provide consultation and education to the IDG on spiritual care.
  • Provide funeral or memorial services for patient’s as requested.
  • Assume responsibility for self-development by continually striving to improve his/her Spiritual practice through formal education, attendance at workshop and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
  • Must be flexible to accept other duties needed or assigned by the resident or institution.
  • Performs other duties as assigned by supervisor or management.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:

  • Ordained, commissioned or credentialed according to the practices of an organized religious group OR has a degree in theology or from a seminary.
  • Comfortable in an interfaith setting.
  • Hospice experience preferred.
  • Able to accept different lifestyles, cultures, beliefs, and values.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.

JOB SUMMARY:

The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services while operating in a resident centered environment.

TYPICAL JOB DUTIES:

  • Completes an initial comprehensive psychosocial assessment of patients.
  • Must be an advocate for the resident to resolve crises.

  • Coordinates resident discharge and continuity of care planning.

MINIMUM QUALIFICATIONS:

  • A bachelor’s degree in a Human Services field including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited institution.
  • One year of supervised social work experience in a health care setting working directly with individuals.
  • Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.

The LECOM Institute for Successful Living is currently accepting resumes for a Full Time Ward Clerk / Medical Records Specialist.

Duties may include:

  • Enter physician orders: to include new admission orders, daily new orders from physician, and daily new orders from Therapy
  • Schedule all incoming appointments as ordered by the physician, to include the scheduling of transportation.
  • To include all follow up: Dialysis, family requests (i.e. funerals), wound clinics and any other appointment as ordered by a physician.
  • To ensure the medical summary and any other related paperwork (i.e. scans, x-rays, test results, labs) is prepared in advance appointments.
  • Communicate with front desk and family any appointments - scheduled, changed, or cancelled.
  • Provide instruction to all who will be attending the appointment - proper infection control and social distancing guidelines.
  • Notify staff of patient appointments, destinations, and times to ensure patient is prepared and ready prior to the appointment.
  • Keep track of all inventory sheets from all new admits, and upload into doc library,
  • Keep track of Disposition of medications returned to pharmacy and upload into medical records. Prepare disposition of medications and provide to nursing staff for review and signature’s for accuracy
  • Notify pharmacy, dietary, Laboratory of all resident that are no longer residing at the facility and also room changes.
  • Prepare and print out or down load onto a disk any request for medical records that are ordered or requested by outside entities
  • Psyche Visits, Podiatry, Dental: follow up with Psyche, Podiatry, Dental visits, upload doc in doc library and maintain and accurate visit record of the Psyche records will become the responsibility of the Ward Clerk
  • Prepare for labs that need drawn the day before they are due and have the information and labs slips ready for the third shift to prepare for the lab draws, including tubes and paperwork. Follow up and track lab results to ensure that the physician is aware of the new lab work results every morning before rounds.
  • Physician visits: accountable for maintaining the schedule for physician rounds, providing the Physician with an accurate list of people that need seen, calling the physician 2 weeks before they need seen to inform them that their visit is due.

REQUIREMENTS:

  • HS Diploma or GED
  • Experience in Healthcare or Medical Records, strongly preferred
  • CNA license is a plus, but not required
  • Experience with scheduling, answering phones, computer use is required

QUALIFICATIONS:

SUMMARY: Ensures proper adherence to therapeutic diets as ordered by the physician through maintaining direct patient and resident nutritional care activities in accordance with the approved diet manual and with ADA standards, State and Federal laws, AOA requirements as well as hospital policy.  Is responsible for Quality Assurance/Performance Improvement activities.

DUTIES AND RESPONSIBILITIES:

  • Reviews medical orders for modified diets.
  • Interviews patients and residents to obtain information regarding food habits and preference which will influence the acceptability of the prescribed diet. 
  • Instructs patients and residents, relatives and/or caretakers regarding diet therapy to be applied during and subsequent to hospitalization.  May recommend to the physician diet adjustments regarding consistency, nutritive content and frequency of feeding.
  • Evaluates patient and resident consumption and progress; reports accordingly in their chart. Participates in care planning.  
  • Prior to discharge, offers recipes, suggests methods of preparation and current sources of gathering further information regarding their diet.  May consult with community agencies responsible for home or institutional care.
  • Develops patient and resident menus considering variety, season of the year, availability of foods, food preferences, appearance, taste texture, nutritional and caloric content, food costs and production; makes necessary revisions on a timely basis. 
  • Establishes and maintains a regular and modified diet recipe file to reflect the approved menus.  Inspects food produced and meal assembly to assure that quality, quantity, taste, temperature and appearance standards and directions are met.
  • Plans, organizes and conducts educational programs for Dietary employees, nursing staff, medical interns, residents, students, physicians, other personnel and members of the community. 
  • Is responsible for individual education and direction of new employees in regard to the therapeutic diet restrictions during their job orientation and training into new positions.
  • Is responsible for records and reports concerning number of patients and resident meals served, menus, supplies issued, diet instructions/consultations and others as requested by the Director of Food Services.

QUALIFICATIONS:

Masters or B.S. in Nutrition and Dietetics degree, 1-2 years healthcare experience preferred.

  • Skills: Analytical skills to accurately assess, plan, and evaluate outcomes. Interpersonal skills to effectively interact with a diverse population and collaborate in a team environment. Strong nutrition education/counseling and mentoring skills. Critical thinking skills for sound decision making.
  • Licensure, Certifications, and Clearances: Registered Dietitian with Commission on Dietetic Registration or registration exam eligible with employment contingent upon passing the RD examination within 3 months of date of hire. Maintenance of registration as demonstrated by 75 CEU credits every 5 years. Licensed Dietitian? Nutritionist (LDN) with the Commonwealth of Pennsylvania within 6 months of hire and maintenance of PA-LDN as demonstrated by 30 CEU credits every 2 years.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Obtain authorization according to insurance requirements within time constraints. Enter insurance communication notes with visit authorizations obtained and send to team members.
  • Monitor authorizations for completion and need for continued authorization requests.
  • Follow up authorization process within 24 hours of receiving notification that forms are complete.
  • Maintain an accurate, timely and daily authorization process and corresponding log to track progress.
  • Must be flexible to accept other duties needed or assigned by the resident or institution.
  • Performs other duties as assigned by supervisor or management.

Main Duties and Responsibilities:

  1. Assist in patient care in the exam room as directed by physician
  2. Perform clinical procedures under supervision of physician
  3. Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
  4. Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations

  16. Keep patients in waiting area informed of any unusual delays in physicians’ schedule
