JOB SUMMARY:
The Wound Nurse is responsible for the coordination of wound care for the residents at the LECOM Institute for Successful Living. These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through standard operating procedures of all wounds.
MINIMUM QUALIFICATIONS:
- Must be Licensed as a Practical Nurse in Pennsylvania.
- Wound Care Certification or Ostomy Certification is preferred but not necessary.
- 1-2 years related experience and/or training; or equivalent combination of education and experience.
- Must maintain current certifications in CPR and First Aid.
TYPICAL JOB DUTIES:
- Provide case management for consulting patients providing specific assessment according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patients’ wound care;
- Provides efficient and effective care with the guidance of the Physician during the wound clinic sessions, attends or follows up on all wound clinic appointments where scheduled. Able to manage a high number of patients at the same time with high energy level and organization;
- Collaborates with other health care providers, wound care Physicians, Nursing Leaders and floor staff, Home Health Agencies, Extended Care Facilities, and Wound Clinic Program Director and Clinical Coordinator, regarding resident needs;
- Will participate in the development and establishment of a goal oriented skin care protocol;
- Implementation of the skin care protocol and orders within 24 hours or next business day after admission;
- Order entry of initial LECOM Wound Clinic orders;
- Order entry as needed upon changes in skin care;
- Daily review of skin incidents and wound / ulcer report accuracy;
- Liaison to LECOM Health Wound Clinic for transfer assistance;
- Assist in resident / family / employee teaching and education as needed;
- Review , document and report significant changes as well as quarterly and annual care plans;
- If Patient has an active wound, will participate in multi-disciplinary team care plan meetings;
- If Patient has no active wounds, a wound nurse business card will be provided during the care plan meeting;
- Must provide daily skilled wound care on all patients, as applicable;
- Nursing staff will perform head to toe skin assessments upon admission or next business day after admission and communicate results to wound nurse;
- The wound nurse will assess either upon admission or on next business day after admission if there is any presence of a wound;
- The wound nurse will assess within 5 days of an admission if no wound is identified by the nursing staff in their head to toe assessment;
- and continue performing at least weekly skin assessments;
- Weekly wound care rounds, follow up and documentation.

Job description:
The Registered Nurse (RN) position is responsible for the coordination of clinical activities of the Wound Care Center. Activities include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care as it relates to the specific needs of the patient. Nurses work within the framework of the State Nurse Practice Act and follows Wound Care Centers, INC, and affiliate hospital policies and procedures. The Nurse complies with hospital and nursing standards in the performance of job duties and responsibilities. The Nurse will support the goals and objectives of the center and of the hospital.
DUTIES AND RESPONSIBILITIES:
Direct Patient Care (80%):
- Providing case management for patients
- Providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management.
- Acting as patient advocate in delivery and coordination of patient care.
Planning and Organizing (10%):
- Providing efficient and effective care with the guidance of the physician during clinic sessions.
- Attending the clinics at which you are scheduled and being dependable.
- Managing multiple patients at the same time with high energy and organization.
Collaboration / Communication / Follow-up (10%):
- Collaborating with other health care providers, wound care center physicians, Program Director and Medical Director, regarding clinic and patient needs.
- Effectively communicating with others, both verbally and written.
- Monitoring the Cases of patients for whom the Nurse is responsible.
Education Requirements: Bachelor’s degree (B.A.) from a four-year college or university; or equivalent combination of education and experience. Registered Nurse and State Licensure and Wound Care QUALIFICATIONS:
Certification preferred.
Minimum Experience: One to two years related experience and/or training; or equivalent combination of education and experience.
Language Skills: Must be able to read, write, and speak the English Language in an understandable manner.
Physical Demands: The incumbent in this position typically has to stand approximately 40% of the time, walk 50% of the time, sit 10% of the time. In addition, the person is using touch or feel 80% of the time, and reaches with their arms or hands approximately 80% of the time. This person may have stoop, kneel, crouch 30% of the time, and talk or hear 100% of the time
Working Environment: The incumbent in this position is not required to work under any extreme conditions, but may be required to work around caustic chemicals approximately 5% of the time, and must be able to work around a moderate noise level, such as an office environment.

DUTIES AND RESPONSIBILITIES:
- Supervises staff and oversees the daily activities of each registration area to maintain department standards.
- Plan and schedule work for the staff ensuring adequate coverage and proper distribution of assignments of duties on his/her shift.
- Coordinates coverage for call offs within the department.
- Participates in on-call rotation to ensure adequate and timely problem solving.

- Regularly hold department huddles and meetings. Maintains ongoing communication with staff to review programs, provide feedback, discuss new developments, and exchange information.
- Member of the Revenue Cycle Team.
- Excel in all functions performed by Registration Clerks in all Registration areas and performs these functions as necessary.
- Perform additional duties as requested.
QUALIFICATIONS:
Minimum Education: High School Diploma or GED required.
Minimum Experience: Minimum of one to three years prior supervisory experience in registration preferred.
Minimum Skills: Demonstrate reliability and a sense of urgency in completing tasks quickly and in a timely manner. Excellent customer service skills. Must show respect for patients, families, and fellow employees in all actions. Competent use of computer systems and software (including Microsoft Office).

Resident Trust Functions
- Maintain petty cash and resident funds
- Monitor cash flow and resident balances
- Make bank deposits as needed
- Dispense funds to residents as requested
- Payout month room and board and vendor payments
- Reconcile accounts monthly
Skills

JOB SUMMARY:
- The Admissions Discharge Nurse assists incoming and discharging residents and their families through each process while coordinating interdepartmental efforts to ensure the entire Admission or Discharge Checklist has been successfully completed, while operating in a resident centered environment.

TYPICAL JOB DUTIES:
- Act as a concierge to new admission residents and families to help walk them through the admissions process.
- Follow the admission and discharge checklist to ensure all necessary steps are completed throughout the process.
- Coordinate with other departments to obtain and share all necessary information with interdepartmental leaders to ensure staff are prepared for each specific admission and discharge.
- Follow up on every admission at the start of the following shift for every Admission whose arrival was during scheduled off hours.
- Act as a concierge to help coordinate the discharge process with residents and families to ensure the process is fully reviewed and completed, including but not limited to coordinate the resident meetings with social work, therapy and nursing.
- Must perform duties as a LPN or RN as licensed on an as needed basis throughout the course of the workday or work week to meet the needs of the residents.

The Radiologist provides diagnostic and interventional imaging services, including image interpretation, procedure performance, and consultative support. The role ensures timely, accurate reports, collaborates with referring clinicians, and supports radiation safety and quality initiatives.
Key Responsibilities:
Clinical Imaging:
- Interpret radiographic, CT, MRI, ultrasound, and nuclear medicine studies per privileges.

Quality & Safety:
- Promote ALARA principles and radiation safety.

Licensure & Certifications:
- Unrestricted Pennsylvania medical license (or eligibility).
- Board certification or eligibility in Diagnostic Radiology.
- BLS/ACLS as required by privileging entity.
Experience & Skills:
- Proficiency with PACS/RIS and voice recognition reporting.

The LECOM Institute for Successful Living is now accepting applicants for its Certified Nurse Aide Training program.
Applicants selected for the LECOM CNA Training program will receive their training at no cost. LECOM will also cover the cost for the licensing exam (terms apply).
Trainees are offered a Full Time CNA position within one or more of our skilled nursing facilities, contingent upon successful completion of the training program and the final exam, with a starting pay rate of $17.50/hr, plus any applicable shift differential.
About the Class
The LECOM CNA Training program is 4 weeks in duration, with a total of 120 hours of classroom and clinical training. The first two weeks of training will be classroom hours from 8:00 AM - 2:30 PM. While the next 2 weeks consist of clinical hours from 7:00 AM- 1:30 PM. The classes are held at LECOM at Village Square, located at 149 W 22nd St, Erie PA.
*Candidates who can work Full Time 2nd shift, 3rd shift, or 12 hour overnight shift upon graduation of the program will be given preference for admittance. Attendance during the training program is mandatory.
Qualifications
- High School Diploma or GED equivalent, strongly preferred
- Applicants must complete a pre-employment physical and criminal background check prior to beginning their training
- Must have reliable transportation to and from the various training locations
