LECOM Nursing and Rehabilitation, a part of the LECOM Institute for Successful Living, is accepting applications and/or resume submissions for a Full Time COTA.
Under supervision of an Occupational Therapist, the Occupational Therapy Assistant administers medically prescribed occupational therapy treatment for patients suffering from injuries and muscle, nerve, joint and bone disease, to restore function, relieve pain, and prevent disability by performing a numbered of required duties for the LECOM Institute for Successful Aging, and other LECOM Health entities, in accordance with current applicable federal, state and local standards, regulations, and guidelines to assure that the highest degree of quality care is rendered and LECOM Health’s standard of excellence is achieved.
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Review physician orders, patient‘s condition, and medical records;
- Review data from the Occupational Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
- Review effective treatment strategies for areas of Activities of Daily Living and Independent Living Skills with the Occupational Therapist;
- Monitor and communicate patient response to occupational therapy treatment;
- Attend meetings in regards to patient care such as care plan meetings, welcome meetings and daily huddles on an as needed basis;
- Assist the Occupational Therapist with patient at discharge including patient and family instruction in safety, mobility, and function;
- Modify Occupational Therapy treatment and plan of care according to patient’s ability and response as directed by the supervising Occupational Therapist;
- Instruct patient and family in treatment procedures to be continued at home;
- Record treatment, response, and progress in the electronic medical record. Documentation of patients care and services will adhere to accepted regulatory, corporate and professional guidelines;
- Maintain compliance with appropriate therapy minutes, therapy intensity, and productivity standards on daily basis;
- Adhere to all HIPPA procedures and guidelines in accordance with LECOM Health’s standard of excellence;
- Work as scheduled, ready to devote full attention and energy to the important work of LECOM Health;
- Educate the patient and/or caregivers to enhance functional independence;
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications and establishing personal networks;
- Comply with federal, state and local legal and professional requirements;and
- Accept other duties as needed/assigned for the Institution’s needs.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:
Required: Occupational Therapy Assistant degree from an accredited institution required. Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety. CPR and Pennsylvania license as an Occupational Therapy Assistant required.

JOB SUMMARY:
The Wound Nurse is responsible for the coordination of wound care. These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through standard operating procedures of all wounds.
MINIMUM QUALIFICATIONS:
- Must be Licensed as a Registered Nurse in Pennsylvania.
- Wound Care Certification or Ostomy Certification is preferred but not necessary.
- 1-2 years related experience and/or training; or equivalent combination of education and experience.
- Must maintain current certifications in CPR and First Aid.
TYPICAL JOB DUTIES:
- Provide case management for consulting patients providing specific assessment according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patients’ wound care;
- Provides efficient and effective care with the guidance of the Physician during the wound clinic sessions, attends or follows up on all wound clinic appointments where scheduled. Able to manage a high number of patients at the same time with high energy level and organization;
- Collaborates with other health care providers, wound care Physicians, Nursing Leaders and floor staff, Home Health Agencies, Extended Care Facilities, and Wound Clinic Program Director and Clinical Coordinator, regarding patient needs;
- Will participate in the development and establishment of a goal-oriented skin care protocol;
- Implementation of the skin care protocol and orders within 24 hours or next business day after admission;
- Order entry of initial LECOM Wound Clinic orders;
- Order entry as needed upon changes in skin care;
- Daily review of skin incidents and wound / ulcer report accuracy;
- Liaison to LECOM Health Wound Clinic for transfer assistance;
- Assist in patient / family / employee teaching and education as needed;
- Review, document and report significant changes as well as quarterly and annual care plans;
- If Patient has an active wound, will participate in multi-disciplinary team care plan meetings;
- If Patient has no active wounds, a wound nurse business card will be provided during the care plan meeting;
- Must provide daily skilled wound care on all patients, as applicable;
- Nursing staff will perform head to toe skin assessments upon admission or next business day after admission and communicate results to wound nurse;
- The wound nurse will assess either upon admission or on next business day after admission if there is any presence of a wound;
- The wound nurse will assess within 5 days of an admission if no wound is identified by the nursing staff in their head-to-toe assessment;
- and continue performing at least weekly skin assessments;
- Weekly wound care rounds, follow up and documentation.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Work at the direction of the Director of Therapy and other senior administrative personal;

- Administers therapy treatments by giving massages; initiating traction; applying physical agents; utilizing hydrotherapy tanks and whirlpool baths, moist packs, ultraviolet and infrared lamps, and ultrasound machines; directing treatments given by aides, technicians, and assistants;
- Evaluates effects of therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modifications;
- Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences;
- Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs;
- Documents patient care services by charting in patient and department records;
- Maintains patient confidence and protects hospital operations by keeping information confidential;
- Maintains safe and clean working environment by complying with procedures, rules, and regulations;
- Protects patients and employees by adhering to infection-control policies and protocols;
- Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs;

Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety. OSHA 10 certification. CPR, first responder with current licensure and certification in specified discipline required.
*Experienced Physical Therapist Assistants may also be considered.
Preferred: Graduate level degree. Certificate in Infection Control (CIC) and/or Certified Safety Personnel (CSP or ASP). OSHA 30 Certification. Basic life-support instructor certification. Advanced Cardiac Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) Certified.

- Serves as a resource person to residents, families, physicians and staff in interpreting department policy.
- Assists DON in aspects of marketing including, but not limited to, developing positive relationships with the community, residents, and families. Providing tours and attending marketing events will also be required.
- Assists in the negotiation of approval for resident stay and rates with payers.
- Serves as an educator for residents, families, referral sources, payers, and case managers.
- Participate in precertification, preadmission screenings, outcome data, and review of records.
- Assists in the coordination of admissions and discharges.
- Conducts resident interviews for social, personal, and medical history.
- Assists residents with the activities of daily living including, but not limited to; bathing, dressing, dining, and transportation.
- Assists in the care planning process including implementation and accurate and timely documentation.
- Will perform as the Acting or Interim D.O.N. in the absence of the D.O.N.
- Must be flexible to accept other duties as needed or assigned by the resident or institution.
- Performs other duties as assigned by supervisor or management.

- Reads prescription, measures arterial blood gases, and reviews patient information to assess patient condition and determine requirements for treatment, such as type and duration of therapy, and medication dosages. Determines most suitable method of administering inhalants, precautions to be observed, and modifications which may be needed that will be compatible with physician's orders.
- Monitors patient's physiological responses to therapy, such as vital signs, arterial blood gases, and blood chemistry changes. Performs bronchopulmonary drainage and assists patient in performing breathing exercises. Performs pulmonary function tests to be used by physician in diagnosis of case. Consults with physician in the event of adverse reactions.

Job Summary:
To register patients who are unscheduled for outpatient services in compliance with the LECOM Health Corry Memorial Hospital policies and procedures.
Characteristic job tasks and responsibilities:
- Register patients and update existing patient demographics by collecting detailed information including personal and financial information along with correct system information.
- Confirm and verify insurance benefits with the appropriate carrier via eligibility verification and/or appropriate insurance websites.
- Scan insurance card, driver’s license and order(s).
- Print face sheet(s), labels and apply arm bands on patients.
- Validate physician’s order for all appropriate information.
- Contact physician’s office if there is no order on file/or need new standing order.
- Collect any co-payments due.
- Complete all applicable forms and necessary communications while obtaining signature for consent to treatment and financial responsibility, HIPAA and Photo Consents.
- Notify departments of patient’s arrival.
- Direct patients to appropriate department for testing.
- File orders in drawer alphabetically as they come in from fax machine.
- Purge orders from drawer that are over 6 months old; verify if testing was done; send to physician’s office to let them know patient did have done at our facility.
- Follow up calls made to Physician’s offices if information not received.
- Look up Physician information if ordering physician is not in the system and add it; Give information to Special Projects Personnel.
- Late Add Ons are registered if not done by pre-reg/insurance verification.
- Assist with other tasks as assigned by Patient Accounting/Registration Manager.

- A high school diploma and completion of the Board approved 120-hour program.
