The Dietary Manager will report to the facility administrator in regards to all operations within the Culinary Department including, but not limited to; hiring, disciplinary actions, scheduling, cost controls, item specifications, menu development, sanitation standards, and record keeping
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Knowledge, skills, attitude, and energy to consistently provide resident centered care.
- Develops friendly and courteous relationships with residents and co-workers.
- Maintains confidentiality of all residents’ personal information.
- Prepares food and drink items using standard formulas or following directions.
- Explains and guides residents in regards cost and contents of their selections and provides alternative selections available.
- Puts away, replenishes, and rotates stock to ensure older items are used first.
- Ensures health guidelines and company sanitation policies are followed by employees and guests in all dining areas.
- Controls conduct of employees and residents in a friendly manner to prevent loss, theft, spills and cross contamination.
- Maintains control of product flow and employees.
- Performs cleaning duties while keeping both front and back café areas clean and free of debris throughout the work day.
- Must perform as a Universal Worker being cross-trained in housekeeping and personal care functions.
- Must be flexible to accept other duties as needed or assigned by the resident or institution.
- Performs other duties as assigned by supervisor or management.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:
- 3-5 Years of Culinary Management Experience.
- Healthcare experience preferred.
- Must have a High School Diploma or GED.

JOB SUMMARY:
- The Admissions Discharge Nurse assists incoming and discharging residents and their families through each process while coordinating interdepartmental efforts to ensure the entire Admission or Discharge Checklist has been successfully completed, while operating in a resident centered environment.
MINIMUM QUALIFICATIONS:
- Must be licensed as a practical nurse or registered nurse in Pennsylvania.
- Must maintain current certifications in CPR and First Aid.
TYPICAL JOB DUTIES:
- Act as a concierge to new admission residents and families to help walk them through the admissions process.
- Follow the admission and discharge checklist to ensure all necessary steps are completed throughout the process.
- Coordinate with other departments to obtain and share all necessary information with interdepartmental leaders to ensure staff are prepared for each specific admission and discharge.
- Follow up on every admission at the start of the following shift for every Admission whose arrival was during scheduled off hours.
- Act as a concierge to help coordinate the discharge process with residents and families to ensure the process is fully reviewed and completed, including but not limited to coordinate the resident meetings with social work, therapy and nursing.
- Must perform duties as a LPN or RN as licensed on an as needed basis throughout the course of the workday or work week to meet the needs of the residents.

- Will maintain daily staffing sheets and adjust them appropriately for accuracy and store them for record keeping purposes;
- Will complete any necessary reporting of schedules or hours worked as needed;

JOB SUMMARY:

The ideal candidate will also perform selected skilled actions in the provision of curative, rehabilitative, palliative or preventative nursing care. · Assists the Registered Nurse in carrying out the plan of care.

Job Summary
The LECOM Institute for Successful Living has an immediate opening for a full-time Nurse Aide Training Program Instructor/Educator (NATCEP). Candidates must possess a valid PA nursing license (RN or LPN). The selected individual will serve as an instructor in a regular classroom and clinical setting, presenting instructions in a state approved Nurse Aide Training Program. The position follows the student from application, through the courses, and provides support through the state examination process, but also acts as an educator for existing and previous Nurse Aide graduates.
Qualifications and Skills

JOB SUMMARY:
The Wound Nurse is responsible for the coordination of wound care for the residents at the LECOM Institute for Successful Aging. These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through standard operating procedures of all wounds.
MINIMUM QUALIFICATIONS:
- Must be Licensed as a Practical Nurse in Pennsylvania.
- Wound Care Certification or Ostomy Certification is preferred but not necessary.
- 1-2 years related experience and/or training; or equivalent combination of education and experience.
- Must maintain current certifications in CPR and First Aid.
TYPICAL JOB DUTIES:
- Provide case management for consulting patients providing specific assessment according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patients’ wound care;
- Provides efficient and effective care with the guidance of the Physician during the wound clinic sessions, attends or follows up on all wound clinic appointments where scheduled. Able to manage a high number of patients at the same time with high energy level and organization;
- Collaborates with other health care providers, wound care Physicians, Nursing Leaders and floor staff, Home Health Agencies, Extended Care Facilities, and Wound Clinic Program Director and Clinical Coordinator, regarding resident needs;
- Will participate in the development and establishment of a goal oriented skin care protocol;
- Implementation of the skin care protocol and orders within 24 hours or next business day after admission;
- Order entry of initial LECOM Wound Clinic orders;
- Order entry as needed upon changes in skin care;
- Daily review of skin incidents and wound / ulcer report accuracy;
- Liaison to LECOM Health Wound Clinic for transfer assistance;
- Assist in resident / family / employee teaching and education as needed;
- Review , document and report significant changes as well as quarterly and annual care plans;
- If Patient has an active wound, will participate in multi-disciplinary team care plan meetings;
- If Patient has no active wounds, a wound nurse business card will be provided during the care plan meeting;
- Must provide daily skilled wound care on all patients, as applicable;
- Nursing staff will perform head to toe skin assessments upon admission or next business day after admission and communicate results to wound nurse;
- The wound nurse will assess either upon admission or on next business day after admission if there is any presence of a wound;
- The wound nurse will assess within 5 days of an admission if no wound is identified by the nursing staff in their head to toe assessment;
- and continue performing at least weekly skin assessments;
- Weekly wound care rounds, follow up and documentation.

Business office Manager Duties
Accounts Payable Functions
- Review vendor statements, code invoices, distribute to department heads and Administrator their review and approval, and forward to Home Office for payment.
- Help resolve vendor disputes.
Accounts Receivable Functions
- Insurance verification
- Keep records of resident room, level of care, and ancillary charges.Update the daily census activity on the Census tracking log
- Gather census information from admissions and reported changes during the morning “stand up” meeting
- Ensure patient days and payers matchIssues that requiring follow up should be noted on the Census tracking log and resolved within 24 hoursMonday – Friday. Weekend activity is validated on Monday.
- Collect checks, deposit payments and forward to the Richter
- Collect, sort and make copies of all walk-in and mailed paymentsRecord payments on the Daily Cash Receipt log
- Upload with copies of corresponding checks on the corporate network shared driveFile original copies at the facility for future reference

- Forward ancillary charges to the CBO
- Including Daily responsibilities to Notify ancillary service vendors of new admissions. Information sent to vendor includes the following however varies based on services provided including Facesheet, Medication listing and Physician order
- Weekly duties to Fax or email updated census information to the ancillary service vendors.
- Review each invoice for billing accuracy
- Residents listed were provided services for the date range captured on the invoice
- Analyze Late charges and adjustments
- Identify and highlight residents with late charges
- Note on invoice that each late charge has been reviewed
- Notify Billing Specialist of last charges
- Scan and save a copy of vendor invoices on the corporate shared drive for review by billing team. Forward original invoices to accounts payable for payment processing.
- Referral Medicaid Pending residents to Medicaid team
- Monthly AR account review with Administrator
- Monthly Triple Check meeting
- Assist in resolving customer financial disputes
- Timely reporting and month-end close
- Cash receipt entry and reconciliation
- Collection and problem resolution for difficult accounts
- Third party billing including Medicare, Medicaid, and Insurance Companies
Resident Trust Functions
- Maintain petty cash and resident funds
- Monitor cash flow and resident balances
- Make bank deposits as needed
- Dispense funds to residents as requested
- Payout month room and board and vendor payments
- Reconcile accounts monthly
Skills
- Knowledge of human resource/payroll procedures and accounting procedures
- Knowledge of Medicare and Medicaid billing requirements
- Possess excellent organizational skills
- Able to multi-task
- Interact with residents, resident family and vendors
- 3-5 years of experiences with nursing home billing
- Able to use standard office equipment and have basic computer skills (including Excel, Word, Outlook)
- Ability to interpret and apply departmental procedures
- Makes independent decisions
- Ability to problem solve as appropriate

Main Duties and Responsibilities:
- Assist in patient care in the exam room as directed by physician
- Perform clinical procedures under supervision of physician
- Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
- Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations

- Keep exam rooms and medication storage room stocked with adequate medical supplies, maintain instruments, prepare sterilization as required
- Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
- Triage for urgency and process messages from patients and front office staff to physicians
- Provide response to patient inquiry and concerns with attention to confidentiality
- Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, cold sterilization fluid change, etc.)
- Keep patients in waiting area informed of any unusual delays in physicians’ schedule

The primary purpose of the LNAC is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Pennsylvania and the policies and goals of our facility. We strive to provide the best clinical outcomes for our residents, and the LNAC is a valuable member of the residents' Care Plan Team. Time management and attention to detail are crucially important to this role.
Job Responsibilities:
- Conduct or coordinate the interviewing of each resident for the resident's assessment
- Evaluate each resident's condition and pertinent medical data to determine the need for special assessment activities or need to amend the admission assessment
- Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies
- Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment function and that they are aware of the penalties, including civil money penalties, for false certifications
- Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition
- Coordinate the review and revision to the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status
- Sign and date the assessment instrument to certify its completion
- Responsible for transmission of resident information to the state per requirements
Education and Qualifications:
- Graduate of an accredited Practical Nursing program required
- Licensure and registration by the PA Board of Nursing Examiners as a Licensed Practical Nurse required
- Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred.
- Previous LNAC experience preferred.
