Main Duties and Responsibilities:
- Assist in patient care in the exam room as directed by physician
- Perform clinical procedures under supervision of physician
- Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
- Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations
- Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit. Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record. (pre-visit planning/chart prep)
- Schedule tests, surgical procedures and coordinate referrals to provide continuous care for patients
- Obtain all necessary authorizations as mandated by patient’s insurance
- Track referrals for the practice to completion (report/result received and patient notified)
- Perform lab draws, EKG’s and administer injections.
- Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician
- Keep exam rooms and medication storage room stocked with adequate medical supplies, maintain instruments, prepare sterilization as required
- Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls
- Triage for urgency and process messages from patients and front office staff to physicians
- Provide response to patient inquiry and concerns with attention to confidentiality
- Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, cold sterilization fluid change, etc.)
- Keep patients in waiting area informed of any unusual delays in physicians’ schedule
- All other duties as assigned
Other Duties:
- Assist in answering phone, scheduling appointments, making reminder calls as well as calling no shows
- Be familiar with and implement all office policies and procedures
- Attend all office meetings and training sessions
- Understand and adhere to the professional policies and standards in the Employee Handbook

The LECOM Institute for Successful Living is now accepting applicants for its Certified Nurse Aide Training program.
Applicants selected for the LECOM CNA Training program will receive their training at no cost. LECOM will also cover the cost for the licensing exam (terms apply).
Trainees are offered a Full Time CNA position within one or more of our skilled nursing facilities, contingent upon successful completion of the training program and the final exam, with a starting pay rate of $17.50/hr, plus any applicable shift differential.
About the Class
The LECOM CNA Training program is 4 weeks in duration, with a total of 120 hours of classroom and clinical training. The first two weeks of training will be classroom hours from 8:00 AM - 2:30 PM. While the next 2 weeks consist of clinical hours from 7:00 AM- 1:30 PM. The classes are held at LECOM at Village Square, located at 149 W 22nd St, Erie PA.
*Candidates who can work Full Time 2nd shift, 3rd shift, or 12 hour overnight shift upon graduation of the program will be given preference for admittance. Attendance during the training program is mandatory.
Qualifications
- High School Diploma or GED equivalent, strongly preferred
- Applicants must complete a pre-employment physical and criminal background check prior to beginning their training
- Must have reliable transportation to and from the various training locations

JOB SUMMARY:
Provide Medical Social Services to the patient, family and/or bereaved according to the Plan of Care.
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the program. This includes emotional, social, financial, and environmental resources and to identify appropriate psychosocial problems, interventions, and goals for the patient’s plan of care.
- Development and implementation of the Bereavement Plan of Care for the bereaved following the hospice patient’s death. The Bereavement Plan of Care is developed using: The bereavement assessment, the risk assessment, Input from the IDG re: bereavement issues during care of the patient as well as at the death visit, and Interaction with the bereaved following the patients death.
- Participate in IDG meetings and the development of the patient’s/bereaved’ s plan of care.
- Observe, record and report changes on the patient’s emotional and social factors that are being affected by the patient’s illness and his/her need for care and his/her response to treatment.
- Maintain and submit written clinical records as deemed by Agency, including the initial evaluation, the care plan and daily notes.
- Evaluate the patients and family’s response to, and effectiveness of, the medical social work intervention.
- Confirm, on a weekly basis the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits.
- Participate in IDG and QAPI meetings and activities.
- If the patient/family declines Social Work services, the Social Worker continues to offer support to the IDG in it’s care of the patient and to monitor the patient/caregiver’s evolving needs.
- Participate in discharge planning, as needed.
- Assume responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
- Act as a consultant to the members of the health team, assists them in understanding the social, emotional, and environmental factors related to the patient’s health problems.
- Help patients to utilize the resources of their families and the community.
- Participate in memorial services, grief support groups, community education, preparation of mailings, bereavement calls, bereavement visits, and other services when deemed necessary.
- Provide referrals to community resources and professional services when deemed necessary.
- Must be flexible to accept other duties needed or assigned by the resident or institution.
- Performs other duties as assigned by supervisor or management.
MINIMUM QUALIFICATIONS:
- Master’s Degree from a School of Social Work accredited by the Council on Social Work Education OR
- Bachelor's degree in Social Work from an institution accredited by the Council on Social Work Education along with one year of social work experience in a healthcare setting and will be supervised by an MSW.
- Required state licensure or certification.
- If BSW, one year of social work experience mandatory; if MSW, on year of social work experience preferred.
- Home Health and/or Hospice experience preferred
- Must be a licensed driver with an insured automobile in good working order.
- The ability to communicate well, both verbally and in writing.

We have a group of qualified professionals who pride themselves on delivering evidence based medicine. Knowledgeable support staff are available to assist you with questions regarding scheduling, medication refills, etc. The support staff functions as a liaison between the patients and the psychiatrists. As an affiliate of the Lake Erie College of Osteopathic Medicine (LECOM), our practice is engaged in the education of future behavioral health providers. Fellows, residents, and students work closely with care team members to gain specialized knowledge in the clinical practice setting.
This role is tasked with providing a full range of patient care responsibilities for a busy outpatient mental health center. The BH Clinical Assistant I guarantees that each patient visit and the logistical arrangements surrounding the visit are carried out with courtesy, professionalism, and compassion, while assisting the physician in patient care and education. The Clinical Assistant may work with more than one specific physician based on clinic needs each day as well as willingness to work at both LIBH settings. Flexibility in scheduling is preferred as we staff our offices M-Th 8-6:30pm, Friday 8-5. Experience working in behavioral health and/or outpatient office setting is highly preferred.
Main Duties and Responsibilities:
Direct Patient Care
· Greet patients warmly, escort patients to exam rooms.
· Measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record.
· Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed documents.
· Assist in patient care in the exam room as directed by physician.
· Perform clinical procedures under supervision of physician.
· Perform screenings per provider guidelines.
· Perform lab draws and administer injections.
· Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician.
· Keep patients in waiting area informed of any unusual delays in physician’s schedule.
Administrative Patient Care
· Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit.
· Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record.
· Schedule appointments, tests, and procedures to provide continuous care for patients.
· Support the obtaining of all necessary authorizations as mandated by patient’s insurance.
· Process prescription refills, and direct/instruct patients and/or family members regarding prescription and treatment instructions according to provider guidelines and orders.
· Aid in coordination and administration of long acting injectables.
· Assist with coordination of Clozaril patient care when requested by Office Lead and/or Clinic Director.
· Support the tracking of patient appointments, tests, and procedures through completion and maintain documentation of efforts in EHR.
· Keep exam rooms and medication storage areas stocked with adequate medical supplies.
· Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls.
· Triage for urgency and process messages from patients and front office staff to physicians.
· Provide response to patient inquiry and concerns with attention to confidentiality.
· Document all patient interactions and contact in the patient chart.
· Ensure daily documentation meets standards, regulations, policies, and practices.
· Keep appropriate records within patient’s EHR chart.
· Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, etc.)
· All other duties as assigned
Other Duties:
1. Assist in answering phone, scheduling appointments, and calling no-shows.
2. Be familiar with and implement all office policies and procedures.
3. Clean equipment, treatment spaces, etc. between visits to maintain infection control as needed.
4. Attend all office meetings and training sessions on time.
5. Perform a variety of routine clerical duties including but not limited to filing, copying, faxing, sorting mail and posting, etc.
6. Understand and adhere to the professional policies and standards in the Employee Handbook.
7. Act as the Goodwill ambassador for the clinic, the hospital and the LECOM Health System by promoting awareness to the patient of all physicians and services LECOM Health provides.
8. Provide clinical coverage at other locations when needed.
Skills/Knowledge:

· Maintain CPR/First Aid certification
· Must be able to obtain Act 33/44 and FBI clearances upon hire.
· Must complete Mandated Reporter training within 30 days of hire.
· Must complete Crisis Management Training upon hire.

Duties and Responsibilities:
Confidently utilize the Telephone System to answer, connect, and transfer calls.
Respond efficiently and calmly to all hospital emergency calls (such as fire alarms, and emergency codes) by notifying appropriate personnel, and overhead paging, following established procedures.
Provide information to callers, visitors and Health System staff within the limits of HIPAA.
Speak clearly, politely and respectfully to patients, visitors, and Health System staff when conveying information using correct grammar.
Remain calm and professional when dealing with difficult situations; receiving complaints and taking action to remedy the complaint; and treating the customer with respect.
Contact physicians and other Hospital personnel as appropriate according to established procedures.
Expedite calls by using cell phones, voice paging as required.
Complete tasks as assigned by the Team Leader.

SUMMARY:
The Mental Health Worker (MHW) conducts mental health intakes and assessments as well as coordinates and provides care that is safe, timely, effective, efficient and client-centered. This position requires the ability to provide intensive case coordination services to individuals with an emotional disturbance or mental illness. The position requires the ability to design and direct the implementation of initial treatment plans through the LECOM Behavioral Health network.
Responsibilities include providing flexible scheduling and service options, assessing strengths and therapeutic needs of the patient, formulating clinical impressions based on biopsychosocial assessment process and assigning diagnoses. Must be willing and able to provide both face-to-face and telehealth services to patients. Must also participate in clinical supervision, case conceptualization, clinical documentation review, progress monitoring, treatment planning, crisis planning, safety planning, discharge planning, and professional development opportunities.
MAIN DUTIES AND RESPONSIBILITIES:
- Respond to patient inquiries in a professional and respectful manner.
- Assists potential patients with their questions and issue resolution.
- Establishes a collaborative relationship with team members, patients/families, and colleagues.
- Respects patients’/families’ choices for intervention and treatment.
- Monitors all methods of correspondence daily and responds to patient and/or stakeholder messages within 24 business hours.

- Assist individuals and families with connecting to interpreters within the community as needed.
Intake & Assessment
- Review Referrals
- Provide Transitional Care Management (TCM) appointment availability and facilitate TCM intakes as directed by clinical supervisor and/or director.
- Complete biopsychosocial assessments of patients as part of an intake interview to identify possible factors contributing to individual’s overall physical and psychological health.
- Assess strengths and therapeutic needs of the individual.
- Help to triage patient need and coordinate assessing the need for sooner appointments with the treating psychiatrist and/or to be the liaison between the patient and the psychiatrist to relay information.
- Assist patients with identifying goals for treatment.
- Coordinate with the treatment team to track treatment and outcomes.
- Oversee the completion of all required paperwork and documentation in the patient’s chart within established departmental timeframes.
- Review new patient intakes and assessments with Clinical Supervisor.
Case Management
- Support new patient registration through initial contact and phone triaging.
- Monitor and manage incoming referrals and care coordination correspondence via facsimile, e-mail, online portal, EMR, phone, etc.
- Schedule and/or help reschedule intakes and mental health assessments.
- Throughout treatment timeframe, provide case management including, but not be limited to coordination of treatment with other service providers, assistance to families in locating needed resources, warm handoffs and referrals to other providers, and patient advocacy.
- Provide individuals and families with information regarding available services as well as discussing the steps and expectations associated with accessing those services.
- Per patient and/or provider request, conduct outreach and follow up with patients between medication management visits.
- Help link patients to formal and informal community resources such as drug and alcohol services, BCM services, transportation services, physical health resources, etc.
- During business hours of operation, provide telephone assistance for behavioral health emergencies, utilizing the patient’s crisis plan and linking patients to higher levels of care as necessary.
- Coordinate with other service providers such as therapist and primary care.
- Work collaboratively with the treatment team and serve as the liaison between patients and medical staff.
- Develop and/or complete written patient documentation/letters as medically necessary and in coordination with approval from the medical director and/or clinic director.
- Support psychotropic medication management as prescribed by medical providers, focusing on adherence and effectiveness of treatment.
- Make outreach calls and consult with psychiatrist between visits to evaluate stability as needed.
- Meet all expectations and deadlines associated with documentation for billable and nonbillable services.
- Facilitate Transitional Care Management calls and request/follow up on receiving hospital or other discharge medical records.
- Maintain all logs associated with tracking work related responsibilities and tasks.
Documentation & Supervision
- Ensure proper documentation is completed for all patient-related care contacts and patient records based upon dialogue with patient, family, and other providers.
- Review progress notes in patient files written by other members of the treatment team to support coordinated care.
- Participate in monthly case reviews with psychiatrist.
- Receive clinical supervision and maintain supervision log.
OTHER DUTIES:
- Participate in training of new team members by offering intake and assessment shadowing opportunities.
- Be familiar with and implement all relevant office policies and procedures.
- Attend all office meetings, clinical supervisions, and training sessions.

SKILLS/KNOWLEDGE:
- MHW qualifications meets one of the following:
o Has a bachelor’s degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the CHEA in a recognized clinical discipline including social work, psychology, nursing, rehabilitation or activity therapies.
o Has a graduate degree in a clinical discipline with 12 graduate-level credits in mental health or counseling from a program that is accredited by an agency recognized by the United States Department of Education or the CHEA.
o Has an equivalent degree from a foreign college or university that has been evaluated by the AICE or the NACES. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.
- Possesses knowledge of Erie County Mental Health Resources and mental health level of care determination.
- Must be able to obtain Act 33/34 and FBI clearances upon hire.
- Must complete Mandated Reporter training within 30 days of hire.
