JOB SUMMARY

The Registered Nurse Supervisor combines leadership and nursing knowledge to assess patient health problems and needs, provide treatments, administer medications, and perform general nursing duties, while operating in a resident centered environment.

TUITION REIMBURSEMENT:

In addition to the new bonus, the RN and LPN Tuition Reimbursement Program was created to assist LECOM Health employees by offsetting a portion of their educational expenses. Registered Nurses or Licensed Practical Nurses may be eligible for tuition reimbursement up to a maximum of $22,000. Inquire for plan details.

MINIMUM QUALIFICATIONS:

  • Must be registered by the Commonwealth of Pennsylvania as a Licensed Registered Nurse.
  • Must maintain current certifications in CPR and First Aid.
  • Demonstrated expertise in analysis, decision-making, time management, oral and written communication and computer use required.

TYPICAL JOB DUTIES:

  • Make daily resident rounds and report status of nursing care to the Director of Nursing.
  • Respond to call bells promptly and assure that call bells are within resident’s reach.
  • Assist residents with the activities of daily living including, but not limited to; bathing, dressing, dining, and transportation.
  • Participate in the care planning process including implementation and accurate and timely documentation.
  • Order, receive, and store medications properly.
  • Competent in using medical equipment including but not limited to mechanical lifts, oxygen concentrators, wheelchairs, geri-chairs, electronic thermometers, scales, sphygmomanometers, stethoscope, electric beds, and pulse oximeter.
  • Check meal trays and nourishment to ensure proper diet.
  • Check medication carts, emergency kits, and exchange keys.
  • Supervise the application and removal of all restraints and devices used to position and enable the resident to function as independently as possible.
  • Supervise the timely completion of MDS assessments and care plans with follow up action as needed.
  • Review resident admissions and compare physician orders with resident’s care plans.
  • Must serve as a teacher and role model to the Resident Concierge staff.
  • Must notify the physician according to current policy related resident status.
  • Assist with analysis of incident and accident investigation reports to determine cause(s) and implement corrective action(s), when appropriate.
  • Must participate in the following: implementation of the Nursing Rehabilitation and Restorative Program; the Quality Assessment and Assurance Program in cooperation with the Quality Assurance Committee; and the interdisciplinary care plan team and weekly Medicare meetings.

The LECOM Parkside Communities are accepting applications and/or resume submissions for a Full Time Temporary Maintenance Technician to work at multiple personal care facilities within Erie County.

The Maintenance Technician provides direct support to the Maintenance and Housekeeping Supervisor while working independently performing general maintenance work on multiple projects inside one or more of LECOM’s skilled nursing facilities. The Technician must be detail oriented, possess excellent computer and time management skills, and the ability to prioritize tasks to ensure that deadlines are met.

  • Collaborates with site personnel, other trades, subcontractors (e.g., scheduling work orders and work activities, identifying project specifications, estimating costs, establishing required delivery dates of equipment and supplies) for the purpose of providing written support, conveying information, and coordinating work assignment activities;
  • Handle confidential and non-routine information;
  • Assist in ensuring the safety of occupants and building users, oversees emergency equipment, supplies, records and OSHA standards;

Accounts Payable Functions

  • Keep records of resident room, level of care, and ancillary charges.Update the daily census activity on the Census tracking log
  • Gather census information from admissions and reported changes during the morning “stand up” meeting
  • Ensure patient days and payers matchIssues that requiring follow up should be noted on the Census tracking log and resolved within 24 hoursMonday – Friday. Weekend activity is validated on Monday.

Qualifications
This is an entry level Plant Operations Specialist position. Employees at this level are expected to perform repairs and improvements and preventative maintenance processes initially under the guidance of experienced staff with the expectation that they will be able to perform these duties with minimal supervision going forward.

Duties and Responsibilities

  1. Perform general repairs and maintenance of the building and equipment.
  2. Perform and document preventative maintenance processes

  5. Perform general landscaping and snow removal as directed.
  6. Repair, spackle, replace and finish drywall to a smooth surface.
  7. Apply paint and other sealants to walls, door frames, and other surfaces.
  8. Lift and convey materials that needed moved in the hospital and outside.
  9. Gathering and disposing of waste materials.
  10. Erect ladders and scaffold and perform work at an elevated level.
  11. Inspect work areas for quality and maintain cleanliness.
  12. Operate tools and equipment in a safe manner according to training, policy, and manufacturer’s instructions.
  13. Receive and complete work orders while maintaining proper records.
  14. Maintain an inventory of tools and supplies related to work being conducted.
  15. Correct unsafe conditions in work area and report unsafe conditions to supervisor.
  16. DON and DOFF appropriate PPE as advised to protect the employee from potentially harmful conditions.

Other Duties as Assigned Other duties can include:

  1. Working at other LECOM facilities, including residential properties.
  2. Filling in other roles when the hospital has urgent needs due to absences, disaster response, or safety issues that might arise.
  3. Working other shifts temporarily to complete tasks that cannot be done during typical work hours.
  4. Any other duties related to meeting the mission and goals of the hospital or Plant Operations.

As an affiliate of the Lake Erie College of Osteopathic Medicine (LECOM), our practice is engaged in the education of future behavioral health providers. Fellows, residents, and students work closely with care team members to gain specialized knowledge in the clinical practice setting. This role is tasked with providing a full range of patient care responsibilities for a busy outpatient mental health center. The BH Clinical Assistant I guarantees that each patient visit and the logistical arrangements surrounding the visit are carried out with courtesy, professionalism, and compassion, while assisting the physician in patient care and education. The Clinical Assistant may work with more than one specific physician based on clinic needs each day as well as willingness to work at both LIBH settings. Flexibility in scheduling is preferred as we staff our offices M-Th 8-6:30pm, Friday 8-5. Experience working in behavioral health and/or outpatient office setting is highly preferred.

Main Duties and Responsibilities:

  • Direct Patient Care · Greet patients warmly, escort patients to exam rooms.
  • Measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record.
  • Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed documents.
  • Assist in patient care in the exam room as directed by physician.
  • Perform clinical procedures under supervision of physician.
  • Perform screenings per provider guidelines.
  • Perform lab draws and administer injections.
  • Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician.
  • Keep patients in waiting area informed of any unusual delays in physician’s schedule. Administrative Patient Care
  • Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit.
  • Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record.
  • Schedule appointments, tests, and procedures to provide continuous care for patients.
  • Support the obtaining of all necessary authorizations as mandated by patient’s insurance.
  • Process prescription refills, and direct/instruct patients and/or family members regarding prescription and treatment instructions according to provider guidelines and orders.
  • Aid in coordination and administration of long acting injectables.
  • Assist with coordination of Clozaril patient care when requested by Office Lead and/or Clinic Director.
  • Support the tracking of patient appointments, tests, and procedures through completion and maintain documentation of efforts in EHR.
  • Keep exam rooms and medication storage areas stocked with adequate medical supplies.
  • Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls.
  • Triage for urgency and process messages from patients and front office staff to physicians.
  • Provide response to patient inquiry and concerns with attention to confidentiality.
  • Document all patient interactions and contact in the patient chart.
  • Ensure daily documentation meets standards, regulations, policies, and practices.
  • Keep appropriate records within patient’s EHR chart.
  • Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, etc.)
  • All other duties as assigned

Other Duties:

  1. Assist in answering phone, scheduling appointments, and calling no-shows.
  2. Be familiar with and implement all office policies and procedures.
  3. Clean equipment, treatment spaces, etc. between visits to maintain infection control as needed.
  4. Attend all office meetings and training sessions on time.
  5. Perform a variety of routine clerical duties including but not limited to filing, copying, faxing, sorting mail and posting, etc.
  6. Understand and adhere to the professional policies and standards in the Employee Handbook.
  7. Act as the Goodwill ambassador for the clinic, the hospital and the LECOM Health System by promoting awareness to the patient of all physicians and services LECOM Health provides.
  8. Provide clinical coverage at other locations when needed.

Provide Medical Social Services to the patient, family and/or bereaved according to the Plan of Care.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the program. This includes emotional, social, financial, and environmental resources and to identify appropriate psychosocial problems, interventions, and goals for the patient’s plan of care.
  • Development and implementation of the Bereavement Plan of Care for the bereaved following the hospice patient’s death. The Bereavement Plan of Care is developed using: The bereavement assessment, the risk assessment, Input from the IDG re: bereavement issues during care of the patient as well as at the death visit, and Interaction with the bereaved following the patients death.
  • Participate in IDG meetings and the development of the patient’s/bereaved’ s plan of care.
  • Observe, record and report changes on the patient’s emotional and social factors that are being affected by the patient’s illness and his/her need for care and his/her response to treatment.
  • Maintain and submit written clinical records as deemed by Agency, including the initial evaluation, the care plan and daily notes.
  • Evaluate the patients and family’s response to, and effectiveness of, the medical social work intervention.
  • Confirm, on a weekly basis the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits.
  • Participate in IDG and QAPI meetings and activities.
  • If the patient/family declines Social Work services, the Social Worker continues to offer support to the IDG in it’s care of the patient and to monitor the patient/caregiver’s evolving needs.
  • Participate in discharge planning, as needed.
  • Assume responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
  • Act as a consultant to the members of the health team, assists them in understanding the social, emotional, and environmental factors related to the patient’s health problems.
  • Help patients to utilize the resources of their families and the community.
  • Participate in memorial services, grief support groups, community education, preparation of mailings, bereavement calls, bereavement visits, and other services when deemed necessary.
  • Provide referrals to community resources and professional services when deemed necessary.
  • Must be flexible to accept other duties needed or assigned by the resident or institution.
  • Performs other duties as assigned by supervisor or management.

MINIMUM QUALIFICATIONS:

  • Master’s Degree from a School of Social Work accredited by the Council on Social Work Education OR
  • Bachelor’s degree in Social Work from an institution accredited by the Council on Social Work Education along with one year of social work experience in a healthcare setting and will be supervised by an MSW.
  • Required state licensure or certification.
  • If BSW, one year of social work experience mandatory; if MSW, on year of social work experience preferred.
  • Home Health and/or Hospice experience preferred
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.
