JOB SUMMARY:

The Visiting Nurse Association of Erie County, a part of the LECOM Institute for Successful Aging, is accepting applications for a Full Time Home Health LPN to provide skilled nursing care under the direct supervision of the Registered Nurse. The ideal candidate will also perform selected skilled actions in the provision of curative, rehabilitative, palliative or preventative nursing care. This position also requires the candidate to be responsible and accountable for making decisions that are based on the individual’s nursing experience and educational preparation.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

· Assists the Registered Nurse in carrying out the plan of care.

· Assists the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician’s orders, excluding infusion care.

· Assists with the preparation, implementation, and continuing evaluation of the patient care plan.

· Recognizes and understands the effects of social and economic problems upon patients and provides for the emotional and physical comfort and safety of patients.

· Helps teach the patient the appropriate self-care techniques.

· Fosters cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.

· Assumes responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.

· Prepares clinical and progress notes, excluding OASIS vistis.

· Observes, records, and reports to the appropriate person patient symptoms, reactions and changes including:

· The general physical and mental condition of patients and signs and symptoms which may be indicative of untoward changes.

· Stresses in human relationships between the patients and staff personnel and between patients and their families and visitors.

· Observes, records, and reports to the appropriate person the general physical and mental condition of patients and signs and symptoms which may be indicative of changes in condition.

· Assists with the rehabilitation of patients according to the medical care plan by encouraging the interests and special aptitudes of the patient and encouraging patients to help themselves using their own capabilities.

· Further assist in rehabilitation by reporting any abnormality in the patient’s range of motion, body mechanics, or body alignment.

· Actively participates in nursing organizations and promotes and participates in in-service programs.

· Must perform as a Universal Worker being cross-trained in other areas including Home Health Aide.

· Must be flexible to accept other duties needed or assigned by the resident or institution.

· Performs other duties as assigned by supervisor or management.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:

  • Must be licensed in Pennsylvania as a Licensed Practical Nurse.
  • One or more years’ experience as an LPN, Home Health or Hospital setting preferred.
  • Must have a criminal background check.
  • Must have current CPR certification.
  • Must have valid Driver’s license and reliable vehicle.

LECOM Nursing and Rehabilitation, a part of the LECOM Institute for Successful Aging, is accepting applications and resumes for a Full Time Respiratory Therapist as we continue to address a growing need for our respiratory services.

SUMMARY:

Our Respiratory Therapists provide care and life support to patients with deficiencies and abnormalities of cardiopulmonary system, under supervision of Physician and by prescription, by performing the following duties.

DUTIES AND RESPONSIBILITIES:

  • Reads prescription, arterial blood gas analysis and reviews patient information to assess patient condition and determine requirements for treatment such as type and duration of therapy, and medication and dosage.
  • Determines most suitable method of administering inhalants, precautions to be observed, and modifications which may be needed that will be compatible with Physician’s orders.
  • Sets up and operates devices such as mechanical ventilators, therapeutic gas administration apparatus environmental control systems.
  • Operates equipment to ensure specified parameters of treatment such as volume, gas concentration, humidity, and temperature, and to administer medicinal gases and aerosol drugs to patients.
  • Monitors patient’s physiological responses to therapy such as vital signs, arterial blood gases, and blood chemistry changes.
  • Performs bronchopulmonary drainage and assists patient in performing breathing exercises.
  • Observe equipment function and adjusts equipment to obtain optimum results to therapy.
  • Consults with Physician in event of adverse reactions.
  • Maintains patient’s chart that contains pertinent identification and therapy information.
  • Inspects and tests respiratory therapy equipment to ensure equipment is functioning safely and efficiently. Orders repairs when needed.
  • Demonstrates respiratory care procedures to trainees and other health care personnel.

EDUCATION AND/OR EXPERIENCE:

  • Six months to one year related experience and/or training; or equivalent combination of education and experience

REQUIRED CERTIFICATION/LICENSE:

  • Must have current PA license, must have current CPR certification

Summary:

Job Description:

This PGY-2 geriatric pharmacy residency program offers an opportunity to excel in clinical and consultant geriatric pharmacy via interactions with a rich health care community. Learning experiences cover a wide array of geriatric experiences and include: acute care, ambulatory care (longitudinal), long term care (core rotation and longitudinal), inpatient rehabilitation, public health, healthcare administration, academic/teaching, and hospice/palliative care. Electives are available in critical care, infectious disease, and other areas. This program offers a flexible schedule designed to give the resident additional experience in their specific area of interest. The resident will gain experiences in teaching and an optional Medical Masters of Science in Medical Education (MSMed) degree is also offered.

July 1st 2024- June 30th 2025

Site Special Features:

The PGY-2 residency program is in association with a school of pharmacy, a college of medicine, the LECOM Institute for Successful Aging, and with a community/teaching hospital (medical residency/fellowship programs). Main Duties and Responsibilities:

  1. Assist in patient care in the exam room as directed by physician
  2. Perform clinical procedures under supervision of physician
  3. Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record

  14. Provide response to patient inquiry and concerns with attention to confidentiality
  15. Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, cold sterilization fluid change, etc.)
  16. Keep patients in waiting area informed of any unusual delays in physicians’ schedule
  17. All other duties as assigned

The Clinical Liaison is a facilitator to all departments and units within the organization in relation to admissions and providing a continuum of care. The Clinical Liaison is an educator, advocate for the patients, consultant, marketer, negotiator and collaborator.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Collaborates with internal and external case management teams to provide a cost-effective plan for each client.
  • Responsible for interviewing patients and family members to obtain information regarding home environment, family relationships, health history, and personality traits of the patient
  • Serves as a rehabilitation consultant, considering rehabilitation potential, therapy progress, internal admissions teams, discharge plan, and special equipment needs.
  • Manages departmental policies and procedures, Quality Assurance program, safety, and environmental and infection control standards.
  • Provides available resources and education regarding utilization services to clients, family members, and referral sources and external payers.
  • Provides necessary information to facilitate referrals and subsequent admissions.
  • Negotiates approval for a client’s stay and rates with payers on necessary referrals.
  • Manages and completes all required documentation for preadmission screening.
  • Performs on site screenings or out of building record reviews, including telephone preadmission screenings and clinical assessments.
  • Assists the Director of Behavioral Health with the departments marketing efforts and facilitating referrals.
  • Coordinates all transitions from the referring facility or site in a professional and timely manner.
  • Refers and recommends appropriate care facilities for candidates who are denied admission.
  • Conducts other duties, such as marketing events and providing tours.

MINIMUM QUALIFICATIONS:

  • A Nurse, Respiratory Therapist, Social Worker, or skilled clinician, preferred but not necessary
  • Familiarity with the resources available in assessing regulations and the parameters of third-party reimbursement a plus.
  • Excellent assessment skills and understanding of the insurance and managed care arenas.
