JOB SUMMARY:

The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. TYPICAL JOB DUTIES:

  • Completes an initial comprehensive psychosocial assessment of patients.
  • Must be an advocate for the resident to resolve crises.
  • Collaborates with other professionals to evaluate the physical, mental, and social aspects of the resident.
  • Monitor, evaluate, and record residents’ progress according to measurable goals described in treatment and care plan.
  • Modify treatment plans to comply with changes in residents' status.
  • Identify environmental impediments to the residents’ progress through interviews and review of resident records.
  • Counsel residents in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
  • Educate residents and family members on the roles and levels of healthcare, community resources, entitlements, and advanced directives.
  • Arranging for resources/funds to finance medications, durable medical equipment, and other needed services.
  • Coordinates resident discharge and continuity of care planning.
  • Answering phone calls and inquiries from potential patients and their families
  • Providing information about the admissions process and requirements
  • Assisting with the completion of necessary paperwork and documentation
  • Coordinating with the admissions team to ensure a smooth transition for patients
  • Maintaining accurate records of all admissions activities

MINIMUM QUALIFICATIONS:

  • A bachelor’s degree in a Human Services field including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited institution.
  • One year of supervised social work experience in a health care setting working directly with individuals.
  • Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Review physician orders, patient‘s condition, and medical records;
  • Review data from the Physical Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
  • Review effective treatment strategies for areas of strength, endurance, and functional mobility with the Physical Therapist;

MINIMUM QUALIFICATIONS:

  • Physical Therapy Assistant degree from an accredited institution required.
  • Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety.
  • CPR and Pennsylvania license as a Physical Therapy Assistant required.
  • Must also possess and independent supervision license.
  • Valid Driver's license

Job summary:

The main occupational tasks will involve creating images of the heart, cardiac chambers, valves, and vessels for the purpose of assessing heart function and diagnosing heart ailments.

Characteristic job tasks and responsibilities:

  • The Tech will work closely with the physician/cardiologist, often performing the technical aspects of the examinations, evaluating the findings, and reporting them to the physician/cardiologists
  • The ECHO Tech will perform ECHOs on both inpatients and outpatients. He/she will be responsible for interviewing the patients and reviewing their medical records so that he/she may adapt their sonographic techniques as necessity dictates.
  • The Tech will provide patient care, such as preparing the patients for the sonographic imaging and providing them with instruction during the echocardiograph so that the images turn out properly.
  • There will be clerical duties related to completion of the ECHO and submitting information to the physician/cardiologist.
  • The tech must be familiar with preparation and administration of contrast such as Definity.
  • Performs other duties as assigned or required.

MINIMUM QUALIFICATIONS:

  • High School Diploma, GED, or equivalent is preferred
  • Previous customer service or personal care experience is preferred

The Nursing Department Scheduler will actively schedule nursing staff of all positions to meet staffing levels while maintaining appropriate records and documentation. This position will also perform duties to provide resident centered care to patients of the LECOM Institute for Successful Living, and to consistently address and communicate resident needs and concerns.

The Nursing Department Scheduler will also function as a Medical Records Specialist and Ward Clerk, as needed.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Knowledge, skills, attitude, and energy to consistently provide resident centered care;
  • Will be responsible to schedule nursing staff including full time, part time, and PRN staff;
  • Responsible to understand ideal and minimum staffing levels for all positions for all shifts and days, and potential changes in those staffing levels;
  • Will utilize PRN staff availability to fill available shifts on the master schedule;
  • Will contact staff in person, via phone, text and email to schedule fill-ins, pick-ups and adjust scheduling needs;
  • Will constantly communicate schedules with all nursing staff, facility scheduler and Nursing management;
  • Will schedule all new nursing staff with orientation and training shifts;
  • Will work with Nursing Management to review and approve timecards in correspondence with the schedule;
  • Will maintain accurate records of what employees are scheduled, time off, and requests;
  • Will maintain daily staffing sheets and adjust them appropriately for accuracy and store them for record keeping purposes;
  • Will complete any necessary reporting of schedules or hours worked as needed
  • Enter physician orders: to include new admission orders, daily new orders from physician, and daily new orders from Therapy
  • Schedule all incoming appointments as ordered by the physician, to include the scheduling of transportation.
  • To include all follow up: Dialysis, family requests (i.e. funerals), wound clinics and any other appointment as ordered by a physician.
  • To ensure the medical summary and any other related paperwork (i.e. scans, x-rays, test results, labs) is prepared in advance appointments.
  • Communicate with front desk and family any appointments - scheduled, changed, or cancelled.
  • Provide instruction to all who will be attending the appointment - proper infection control and social distancing guidelines.
  • Notify staff of patient appointments, destinations, and times to ensure patient is prepared and ready prior to the appointment.
  • Keep track of all inventory sheets from all new admits, and upload into doc library,
  • Keep track of Disposition of medications returned to pharmacy and upload into medical records. Prepare disposition of medications and provide to nursing staff for review and signature’s for accuracy
  • Notify pharmacy, dietary, Laboratory of all resident that are no longer residing at the facility and also room changes.
  • Prepare and print out or down load onto a disk any request for medical records that are ordered or requested by outside entities
  • Psyche Visits, Podiatry, Dental: follow up with Psyche, Podiatry, Dental visits, upload doc in doc library and maintain and accurate visit record of the Psyche records will become the responsibility of the Ward Clerk
  • Prepare for labs that need drawn the day before they are due and have the information and labs slips ready for the third shift to prepare for the lab draws, including tubes and paperwork. Follow up and track lab results to ensure that the physician is aware of the new lab work results every morning before rounds.
  • Physician visits: accountable for maintaining the schedule for physician rounds, providing the Physician with an accurate list of people that need seen, calling the physician 2 weeks before they need seen to inform them that their visit is due.
