The primary purpose of the LNAC is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Pennsylvania and the policies and goals of our facility. We strive to provide the best clinical outcomes for our residents, and the LNAC is a valuable member of the residents' Care Plan Team. Time management and attention to detail are crucially important to this role.

Job Responsibilities:

  • Conduct or coordinate the interviewing of each resident for the resident's assessment
  • Evaluate each resident's condition and pertinent medical data to determine the need for special assessment activities or need to amend the admission assessment
  • Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment function and that they are aware of the penalties, including civil money penalties, for false certifications
  • Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition
  • Coordinate the review and revision to the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status
  • Sign and date the assessment instrument to certify its completion
  • Responsible for transmission of resident information to the state per requirements

Education and Qualifications:

  • Graduate of an accredited Practical Nursing program required
  • Licensure and registration by the PA Board of Nursing Examiners as a Licensed Practical Nurse required
  • Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred.
  • Previous LNAC experience preferred.

The Clinical liaison will serve as a patient advocate to connect patients and families to services ensuring their comfort. CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

· In collaboration with agency leaders, Develop and initiate call strategy based on expected call volumes set forth by Visiting Doctor offices, hospital discharge desks, Assisted Living facilities and other possible sources of referrals to present Agency information and obtain patient referrals.

· Responsible for HomeHealth and Hospice referral management, including but not limited to initial contact, entry into EMR, review of qualifying criteria, consults as needed, collaborating with Medical Director, coordinating admission, meds, equipment, insurance verification and authorization.

· Assists with daily staff update call and proper communication with clinical and operational staff members.

· Assists with staff orientation as well as in-services for current staff.

· Coordinates the dissemination of patient information to skilled facilities, patient homes, and physicians.

· Provide COP Education to contracted facilities ensuring compliance.

· Analyzes the potential of the company’s service area to determine target markets.

· Visits Doctor offices, hospital discharge coordinators, Assisted Living facilities and other possible sources of referrals to present Agency information and obtain patient referrals.

· Analyzes the company’s organization to determine strengths and weaknesses.

· Analyzes past and current marketing data.

· Analyzes patient/company relationships.

· Develops a marketing plan, identifying priorities and sets a reasonable timetable.

· Implements marketing plan staying within established timetable.

· Reviews and evaluates the analyses and plan on an established basis.

· Must be flexible to accept other duties needed or assigned by the resident or institution.

· Performs other duties as assigned by supervisor or management.

MINIMUM QUALIFICATIONS:

· At least 1 year experience in marketing, nursing, or social work interacting with health agencies/professionals.

· Must have a minimum of 2 years of hospice experience.

· Ability to market and deal tactfully with customers and the community.

· Knowledge of business management, governmental regulations, and CHAP standards

· Ability to develop and implement improved methods of operation.

· The ability to communicate well, both verbally and in writing.

· Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.

· Must be organized and detail oriented.

· Must be able to interact with health professionals at all levels.

Located within five miles of all major Erie hospitals, LECOM Nursing and Rehabilitation provides skilled care in a bright, inviting setting. LECOM Nursing and Rehabilitation offers specialized programs and a range of unique treatments for residents recovering from illnesses, post-accident trauma, chronic disease management, and surgical procedures. SUMMARY:

Responsible for overall facility management, profitability, operations, and direction in all aspects. Accountable for, but not limited to, census development, management of accounts receivable and collections, maximization of Net Operating Income, resident/patient care, state and federal survey compliance, positive employee relations, a positive return on investment, an effective business plan and implementation of core programs. Designated the representative in the facility and community. Follows all policies and procedures. Completes rounds of entire facility premises at least daily to ensure compliance with all policies, procedures and regulations. Hands-on involvement with the Social Services department.

MINIMUM QUALIFICATIONS:
Education and experience equivalent to:

  • A Bachelor’s degree from a CAHME (Commission of Accreditation of Healthcare Management Education) accredited institution in health administration, public administration, long-term care administration, or other relevant fields.
  • Master’s degree preferred.
  • Registered Nurse license preferred.
  • Must have successfully completed and passed the PA Nursing Home Administrator (NHA) exam.
  • Completion of Board Approved internship in a medical setting or 3+ years experienced required.
  • Completion of 800-1,000 hours Administrator-in-Training (AIT) preferred.
  • Must complete required 48 hours of continuing education using web-based training approved by the NAB.

We are looking for a reliable Full Time Transportation Assistant to act as a communication point for emergency and non-emergency calls. You will receive requests, transmit messages and track vehicles.

Summary

We are seeking a skilled Vascular Technologist to join our team at LECOM Medical Center and Behavioral Health Pavilion. In this role, you will be responsible for performing diagnostic vascular ultrasound examinations, contributing significantly to patient care and the overall mission of our facility.

Responsibilities

  • Perform non-invasive vascular ultrasound examinations to assess blood flow and vascular conditions.
  • Collaborate with physicians to interpret ultrasound results and assist in diagnosis.

SUMMARY: Responsible for duties and processes related to medical record retrieval, assembly, maintenance and processing. Responsible for scanning, analyzing and release of information in accordance with organizational timelines/policies and state/federal regulations. The HIM Technician safeguards and protects patient’s right to privacy and maintains confidentiality.

DUTIES AND RESPONSIBILITIES:

1. Assembles charts of all discharged inpatient and outpatient in the prescribed fashion, making sure that each page carries the patient’s name and medical record number. Following the established order of assemble and filing protocol as referenced by facility policy and procedure; completes scanning of all records. After scanning, verify all documents are completed and proper form ID’s are applied.

2. Ensures records are complete and properly stored and/or organized before filing or archiving.

3. When directed, prepares records for purging and/or shredding and may participate in medical record destruction with direct supervision.

4. Examines medical records for current and discharged patients for completeness. Medical records are analyzed for deficiencies as defined by state and federal medical record requirements, and hospital policies. Deficiencies are tagged for completion and entered into the EMR.

5. Responsible for verifying the removal of deficiencies once completed by provider.

6. Tracks unaddressed chart delinquencies, sending notifications to applicable parties at designated points in time as specified by departmental policy.

7. Processes subpoenas, court orders, legal requests and other non-routine requests for information in addition to routine requests.

8. Answers telephones promptly and courteously. Routes calls, takes messages, and provides routine information to customers as appropriate.

9. Communicates cooperatively and constructively with physicians, patients, guests, and members of the healthcare team to ensure accurate and timely completion of processes and continuity of services.

10. Responsible for safeguarding patient records and protecting patients’ right to privacy by ensuring compliance with HIPAA standards and maintaining confidentiality in all phases of work.

11. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

12. Performs other related duties as assigned or requested.

QUALIFICATIONS:

Minimum Education: High school or equivalent

Minimum Experience:

· 1-3 years of office experience required

· Knowledge of the medical record format, physiology and disease process, and understanding of medical terminology preferred. If no experience, strong interest in learning.

Job Summary

The Case Manager/Medical Social Worker is responsible for performing case management, utilization review, quality assurance, and discharge planning.

Responsibilities specific to Case Management includes assessment, identification of specific needs, and social service intervention while in the acute setting. Awareness of services available to patients and their families are an important part of this assessment.

Responsibilities specific to Utilization Review include performing admission and concurrent review, and at times retro-review of all in-patients conforming to Medicare, Medicaid, and/or other Third Party Payer requirements. These review processes may be applicable also to Contracted Managed Care members who are patients in the acute/ extended care units of the hospital. Issuance of non-coverage letters at time of discharge to the acute/extended care patient is also considered part of the discharge planning process as specified by contracted Health Plans.

Responsibilities specific to Quality Assurance include performing surveillance and data collection as directed for trend recognition and development of effective actions/plans.

Education Requirements: Bachelor’s Degree in Social Work Membership of the Stroke team includes mandatory hours of education annually specifically related to diagnosis/assessment and management of acute stroke/ cerebrovascular disease.

Minimum Experience: 2 years clinical experience - preferred

Our Inpatient Rehab Unit LPNs provide professional nursing care under supervision for assigned patients according to established standards and practices.

DUTIES AND RESPONSIBILITIES:

1. Maintains established departmental policies and procedures, quality assessment program, safety, environmental and infection control standards.

2. Cooperates with other personnel to achieve department objectives and maintain good employee relations.

3. Monitors medical condition of patients and reports changes to appropriate personnel.

4. Performs basic nursing procedures including application of simple dressings, administration of medications, and general rehabilitation nursing care.

5. Prepares, hangs, monitors and discontinues I.V. fluids.

6. Demonstrates sensitivity to patient’s comfort and privacy and expresses interest in their progress.

7. Monitors emotional stability of patients and reports changes to physician or charge nurse.

Minimum Experience: Sufficient previous clinical experience in area of assignment

Req. Certification/Registration: Current PA LPN license and CPR certification
