Summary:
- The cook will prepare bulk food items for the various venues while following recipes to be nutritious, attractive, in proper portions while being responsible with resources. Will also be responsible to report problems and take corrective action to address those issues.
- Possesses knowledge, skills, attitude, and energy to consistently provide resident centered care.
- Maintains and promotes ethical conduct of employees, addresses residents in a friendly manner, creating a safe and productive environment.
- Will manage the schedule, food supply and food production duties in the absence of dietary managers.
- Responsible for daily food preparation to prepare food for multiple venues while following recipes.
- Responsible for food production for catering events, off-premises food services including meals on wheels, LECOM Center for Health and Aging, and other customers as assigned.
- Promotes a safe working environment.
- Read and interpret the Resident’s nutritional requirements via paper or digital platform.
- Promote clinical nutrition through the Dietitian’s management of therapeutic diets, nutritional supplements and the food first philosophy while producing accurate food offerings.
- Educates and guides residents in regards to cost and contents of their selections and provides alternative selections available.
- Ensures health guidelines and company sanitation policies are followed by employees and guests in all dining areas.
- Maintain all documentation and logs for sanitation and food safety compliance purposes.
- Assists the Dietary Manager to facilitate departmental auditing in preparation for annual survey.

JOB SUMMARY:
The Kitchen Supervisor will assist the Culinary Director in all operations within the Dietary department including, but not limited to; staff scheduling, disciplinary actions, scheduling, cost controls, sanitation standards, and record keeping.
**The schedule for this position will consist primarily of evening and weekend hours. This is a hands-on supervisory role, in which the candidate is expected to also work side-by-side with their team.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

- Ensures health guidelines and company sanitation policies are followed by employees and guests in all dining areas.
- Controls conduct of employees and residents in a friendly manner to prevent loss, theft, spills and cross contamination.
- Maintains control of product flow and employees.
- Performs cleaning duties while keeping both front and back of kitchen areas clean and free of debris throughout the work day.
- Must be flexible to accept other duties as needed or assigned by the resident or institution.

MINIMUM QUALIFICATIONS:
- Must have a High School Diploma or GED
- Approximately one-year experience in high volume food productions. Must have knowledge of cooking methods and procedures, including various types of preparation and related routines. Familiar with kitchen equipment. ServSafe certification within 6 months.

LECOM Institute of Behavioral Health Outpatient is seeking an enthusiastic professional to join our growing outpatient clinic. Full-time 1st shift. SUMMARY: The BH Clinic Scheduler has an overall responsibility of maximizing accessibility to the clinic by managing the schedules of providers and ensuring patients are seen within recommended clinical timeframes designated by prescribing physicians and CRNPs. The position facilitates timely checkout from the practice and providers superb customer service with patients, caregivers, and collaborating professionals.
MAIN DUTIES AND RESPONSIBILITIES:
- Customer Service
- Respond to incoming clinic calls and messages about scheduling in a professional and courteous manner.
- Clear voicemails at least three times per day and maintain log of time to resolve all scheduling-related messages.
- Assist callers of the clinic with general information about the LIBH practice.
- Protect patients’ rights by maintaining confidentiality of personal and financial information.
- Facilitate efficient and attentive check-out process.
- Notify Clinic Director of urgent patient needs, complaints, and/or safety issues.
- Fulfill receptionist duties during breaks and/or reception staff time off. Scheduling
- Oversight and ownership of medication management and Resident therapy schedule to assure all scheduling parameters are being monitored, schedules are correct, and patients are scheduled in accordance with time frames outlined by providers.
- Maintain updated information on provider schedules ensuring patients are scheduled properly (by age, insurance, capacity, etc.).
- Distinguish the priority of patients’ needs to determine when an immediate slot should be found and offered.
- Assist providers and patient callers with scheduling and rescheduling appointments.
- Proactively coordinate with Clinic Director and team to ensure psychiatric time is fully utilized.
- Maintain a backfill strategy/list for each provider.
- Monitor and document all missed appointments.
- Contact all patients that cancelled or did not show to their appointment to reschedule and document attempts in patient chart.
- Resolve scheduling conflicts as they occur.
- Process and monitor provider PTO and CME requests, as well as the Resident post call calendar, and schedule blocks needed for meetings.
- Liaise and coordinate with Clinic Director, Medical Director, and/or Clinic Operations team about provider schedules, change requests, schedule strategy.
- Electronic Health Record Documentation
- Utilize electronic health record to message staff regarding patient scheduling needs/requests.
- Document in patient chart about specific patient needs, pertinent updates, etc. at the time of scheduling/rescheduling.
- Coordinate with receptionists to reconcile daily encounters when conducting day end review. Clerical · Maintain logs of required data as needed.
- Submit routine reports as requested by Clinic Director, Medical Associates of Erie (MAE) and/or MCH.
OTHER DUTIES

- Maintain patient confidentiality as required by the MCH policies and HIPAA.

SKILLS/KNOWLEDGE
- Required o High school diploma or equivalent o Front desk and/or customer service experience.
- Preferred o Scheduling experience. o Formal clerical or secretarial skills training program outside of high school.
- Behavioral health experience helpful, but not required.

Main Duties and Responsibilities:
1. Assist in patient care in the exam room as directed by physician
2. Perform clinical procedures under supervision of physician
3. Escort patients to exam rooms, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record
4. Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed authorizations

SUMMARY:
The Mental Health Professional II (MHP II) conducts mental health intakes and biopsychosocial assessments, provides mental health therapy, and supports intensive case management needs for adults who have serious mental illness. The position requires the ability to design and direct the implementation of treatment plans and collaborate with treatment team members through the LECOM Health network.
Responsibilities include assessing the strengths and therapeutic needs of patients, formulating clinical impressions based on biopsychosocial assessments, assigning diagnoses, collaborating with patients to develop clinically sound treatment plans, providing culturally competent and patient-centered outpatient therapy, and ensuring flexible and accommodating scheduling. The MHP II must be comfortable with and capable of engaging with patients both face-to-face and via telehealth.
Main Duties and Responsibilities:
Customer Service
- Assist potential patients with their questions and concerns.
- Respond to patient inquiries in a professional and respectful manner.
- Establish a collaborative relationship with team members, patients/families, and colleagues.
- Respect patient/family preferences and choices for intervention and treatment.

- Complete biopsychosocial intake assessments with patients to identify factors impacting overall wellness and psychological health.

- Assist patients with treatment goal identification.
- Assess treatment adherence, tolerability, and clinical response using validated rating scales.
- Help to triage patient needs and adjust appointment intervals for therapy and/or the treating psychiatrist as necessary.
- Coordinate with the treatment team to track treatment and outcomes.
- Oversee the completion of required paperwork and documentation in the patient’s chart within established departmental timeframes.
- Review all referrals and triages for clinical appropriateness of outpatient therapy services.
Mental Health Therapy
- Engage in collaborative outpatient therapy services with patients.
- Promote stabilization and progress toward identified treatment goals.
- Formulate appropriate treatment, safety, crisis, and discharge plans.
- Regularly monitor patient progress and reevaluate patient needs.
- Help with system navigation to other service lines or levels of care as necessary.
- Participate in scheduled individual and group supervision.

Case Management
- Provide case management and crisis support services which would include, but not be limited to, coordination of treatment with other service providers, assistance to families in locating needed resources, and patient advocacy.
- Outreach to and engagement of patients between visits as clinically necessary.
- Track referrals to completion and contact patients who have not yet followed up with recommended community resources.
- Help patients gain access to community resources such as: drug and alcohol services, BCM services, transportation services, physical health resources, etc.
- Help in a crisis or emergency and link patients to higher levels of care such as inpatient.
- Coordinate with other service providers such as case managers and primary care.
- Work collaboratively with the treatment team and serve as the liaison between patients and medical staff.
- Support psychotropic medication management as prescribed by medical providers, focusing on adherence and effectiveness of treatment.
- Make outreach calls and consult in between visits with the psychiatrist to evaluate stability.
- Help to coordinate long acting injectable and Clozaril patient care.
Documentation & Supervision
- Ensure proper documentation in patient records based upon dialogue with patient, family and other providers.
- Review progress notes and treatment plans in patient files written by other members of the treatment team.
- Participate in psychiatric case review meetings.
- Participate in clinical supervision and maintain supervision log.
- Maintain professional license.
Other Duties:
- Be familiar with and implement all relevant office policies and procedures
- Attend all office meetings, clinical supervisions, and training sessions
- Provide support to program interns via appointment shadowing and dialoguing to aid in the furtherance of their study and skill practice.
- Understand and adhere to the professional policies and standards in the Millcreek Community Hospital Employee Handbook
- Adhere to obligations under clinic’s mandated reporter policy.
- Adhere to obligations under clinic’s duty to warn policy.

Skills/Knowledge:
- Graduate Degree in mental health field required.
- Current PA LPC or LCSW Licensure (or license eligible) preferred.
- Possesses knowledge of Erie County Mental Health Resources and mental health level of care determination.

- Must be able to obtain Act 33/34 and FBI clearances upon hire.
- Must complete Mandated Reporter training within 30 days of hire.

This role is tasked with providing a full range of patient care responsibilities for a busy outpatient mental health center. The BH Clinical Assistant I guarantees that each patient visit and the logistical arrangements surrounding the visit are carried out with courtesy, professionalism, and compassion, while assisting the physician in patient care and education. The Clinical Assistant may work with more than one specific physician based on clinic needs each day as well as willingness to work at both LIBH settings. Flexibility in scheduling is preferred as we staff our offices M-Th 8-6:30pm, Friday 8-5. Experience working in behavioral health and/or outpatient office setting is highly preferred.
Main Duties and Responsibilities:
Direct Patient Care
· Greet patients warmly, escort patients to exam rooms.
· Measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patient’s electronic health record.
· Interview patients to make sure all information in electronic medical record is complete and current, including obtaining all necessary signed documents.
· Assist in patient care in the exam room as directed by physician.
· Perform clinical procedures under supervision of physician.
· Perform screenings per provider guidelines.
· Perform lab draws and administer injections.
· Provide education to patients, families, and caregivers regarding medication and treatment protocol as instructed by physician.
· Keep patients in waiting area informed of any unusual delays in physician’s schedule.
Administrative Patient Care
· Review medical record for progress notes, specialist templates and diagnostic records prior to patient visit.
· Ensure all related reports, labs and information is filed, if available, in patient’s electronic health record.
· Schedule appointments, tests, and procedures to provide continuous care for patients.
· Support the obtaining of all necessary authorizations as mandated by patient’s insurance.
· Process prescription refills, and direct/instruct patients and/or family members regarding prescription and treatment instructions according to provider guidelines and orders.
· Aid in coordination and administration of long acting injectables.
· Assist with coordination of Clozaril patient care when requested by Office Lead and/or Clinic Director.
· Support the tracking of patient appointments, tests, and procedures through completion and maintain documentation of efforts in EHR.
· Keep exam rooms and medication storage areas stocked with adequate medical supplies.
· Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls.
· Triage for urgency and process messages from patients and front office staff to physicians.
· Provide response to patient inquiry and concerns with attention to confidentiality.
· Document all patient interactions and contact in the patient chart.
· Ensure daily documentation meets standards, regulations, policies, and practices.
· Keep appropriate records within patient’s EHR chart.
· Maintain all logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, etc.)
· All other duties as assigned
Other Duties:

2. Be familiar with and implement all office policies and procedures.
3. Clean equipment, treatment spaces, etc. between visits to maintain infection control as needed.
4. Attend all office meetings and training sessions on time.
5. Perform a variety of routine clerical duties including but not limited to filing, copying, faxing, sorting mail and posting, etc.
6. Understand and adhere to the professional policies and standards in the Employee Handbook.
7. Act as the Goodwill ambassador for the clinic, the hospital and the LECOM Health System by promoting awareness to the patient of all physicians and services LECOM Health provides.
8. Provide clinical coverage at other locations when needed.

SUMMARY: Plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone disease, to restore function, relieve pain, and prevent disability by performing the following duties.
DUTIES AND RESPONSIBILITIES:
1. Reviews Physician’s referral (prescription) and patient’s condition and medical records to determine physical therapy treatment required.
2. Tests and measures patient’s strength, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise treatment.
3. Plans and prepares written treatment program based on evaluation of patient data.
4. Administers manual exercises to improve and maintain function.
5. Instructs, motivates, and assists patient to perform various physical activities and in use of assistant and supportive devices such as crutches, canes, and prostheses.
6. Administers treatments involving application of physical agents.
7. Evaluates effects of treatment at various stages and adjusts treatment to achieve maximum benefit.
8. Administers massage. Administers traction to relieve pain.
9. Records treatment, response, and progress in patient’s chart or enters information into computer.
10. Instructs patient and family in treatment procedures to be continued at home.
11. Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends modification to Orthotist.
12. Confers with Physician and other practitioners to obtain additional patient information, suggests revisions in treatment programs, and integrate physical therapy treatment with other aspects of patient’s health care.
13. Orients, instructs, and directs work activities of assistants, aides, and students.
14. Plans and conducts lectures and training programs on physical therapy and related topics for medical staff, students, and community groups.
15. Perform other duties as assigned or requested.
