PGY-2 Geriatric Pharmacy Resident
Job Description:
This PGY-2 geriatric pharmacy residency program offers an opportunity to excel in clinical and consultant geriatric pharmacy via interactions with a rich health care community. Learning experiences cover a wide array of geriatric experiences and include: acute care, ambulatory care (longitudinal), long term care (core rotation and longitudinal), inpatient rehabilitation, public health, healthcare administration, academic/teaching, and hospice/palliative care. Electives are available in critical care, infectious disease, and other areas. This program offers a flexible schedule designed to give the resident additional experience in their specific area of interest. The resident will gain experiences in teaching and an optional Medical Masters of Science in Medical Education (MSMed) degree is also offered.

The PGY-2 residency program is in association with a school of pharmacy, a college of medicine, the LECOM Institute for Successful Aging, and with a community/teaching hospital (medical residency/fellowship programs). We are located at the border of Lake Erie, a small town with the amenities of a larger city, two hours from Cleveland, Buffalo, and Pittsburgh.
Qualifications:
- Doctorate of pharmacy degree
- Completion of ASHP accredited PGY1 pharmacy residency
- Copy of PGY1 residency certificate presented to residency program director
- Pennsylvania pharmacy licensure (or eligibility for licensure by July 31st, 2024)
- Three letters of recommendations
- Curriculum vitae
- Pharmacy school transcripts
Responsibilities:
- Provide clinical services and comprehensive medication management to patients during all core and longitudinal rotations
- Ensure continuity of care during transitions of care between settings
- Manage and facilitate delivery of medications
- Perform comprehensive medication regimen reviews monthly on a subset of patients in the long term care setting
- Demonstrate leadership skills via interactions with pharmacy technicians, pharmacy interns, and pharmacy students
- Effectively provide education to patients, caregivers, pharmacy students, and the interprofessional healthcare team
- Complete a quality improvement research project and medication use evaluation with poster presentation at an annual pharmacy assembly
