We are seeking a dedicated and compassionate Pharmacist to join our team at the LECOM Senior Living Center. As a Full Time Pharmacist, you will play a crucial role in ensuring the safe and effective medication management for our residents. This position offers the opportunity to work in a collaborative environment where patient care and well-being are our top priorities.

The schedule for this role will be mostly 2nd shift, between the hours of 12pm-10pm, with a weekend rotation.

Responsibilities:

• Review medication orders and ensure accuracy and appropriateness of prescriptions.
• Collaborate with healthcare professionals to develop and implement comprehensive medication management plans.
• Monitor patient health and progress to optimize medication therapies.
• Provide drug information to healthcare staff, residents, and their families as needed.
• Oversee medication distribution processes and ensure compliance with regulatory requirements.
• Conduct periodic reviews and audits to maintain high standards of pharmaceutical care.
• Participate in multidisciplinary rounds and meetings to discuss resident care plans.

Requirements:

• Degree in Pharmacy (Pharm.D. or B.S. in Pharmacy) from an accredited institution.
• Active Pharmacist license in Pennsylvania.
• Previous experience in a skilled nursing facility or long-term care setting preferred.
• Strong knowledge of pharmaceuticals and their indications, contraindications, dosing, side effects, and proper administration.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Commitment to providing exceptional patient care.
