Millcreek Community Hospital, an equal opportunity employer, is looking for an enthusiastic PSYCHIATRIC RESIDENTIAL TREATMENT FACILITY CASE MANAGER with excellent patient care to join our team.

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong commitment to helping clients navigate complex systems and achieve their personal goals. The Case Manager will work closely with clients to assess their needs, develop individualized plans, and connect them with appropriate services.

Duties

  • Conduct comprehensive assessments to identify client needs and strengths including all consents, releases of information, admission psychosocial, and obtaining insurance authorization.
  • Develop, implement, and monitor individualized service plans tailored to each client’s unique circumstances.
  • Provide ongoing support through motivational interviewing techniques to empower clients in their journey.
  • Conducts and documents weekly individual therapy sessions
  • Collaborates with and provides updates to all external parties including community-based family therapy provider, guardians, school, etc.
  • Manages patient schedules
  • Manage crisis situations effectively while maintaining a calm and supportive environment.
  • Facilitates discharges by sending referrals as needed
  • Complete discharge paperwork processes
  • Maintain accurate and up-to-date case records in compliance with organizational policies.

Qualifications

  • Master’s Degree in Social Work/Mental Health Counseling/Related social service discipline required or Child and Adolescent social work with clinical experience preferred.
  • Proven experience in case management or social services preferred.
  • Strong knowledge of workforce development strategies and resources..
  • Experience in crisis management and behavioral health support is essential.
  • Knowledge of addiction counseling practices is a plus.
  • Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
  • Strong organizational skills and attention to detail are required.
  • Ability to work independently as well as part of a collaborative team.
