The LECOM Institute for Successful Aging is currently seeking a Part Time Receptionist at the LECOM Senior Living Center, located at 5535 Peach St, Erie PA, to perform administrative and office support activities for multiple supervisors.

Duties may include fielding telephone calls, receiving and directing visitors, word processing, creating spreadsheets and presentations, and filing. Extensive software skills are required, as well as Internet research abilities and strong communication skills. Sensitivity to confidential matters is required through HIPPA.

**This position will be Part Time. Hours available could consist of weekday hours (between 8:00am-8:00pm) and weekend hours (8:00am-4:00pm) based on facility and/or staffing needs.**

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES (may include, but not limited to):

• Answers the main phone line and transfers calls.
• Distributes mail/packages to residents and employees, collects outgoing mail, and adds postage to outgoing mail.
• Coordinates calendars for the building Administrator
• Responsible for keeping the printer/fax machine in working order by stocking paper, making service calls, and distributing faxes.
• Organizes and creates new resident name tags and welcome gifts
• Coordinates conference room reservations.
• Updates the phone directory with additions/deletions and distributes them appropriately.
• Responsible for communication application administration.
• Coordinates and sits in on the meetings to take minutes.
• Controls administrative office traffic by overseeing the office doorbell.
• Creates Microsoft Office documents for various departments.
• Knowledge, skills, attitude, and energy to consistently provide resident centered care.
• Must perform as a Universal Worker being cross-trained in housekeeping, dietary and personal care functions.
• Must be flexible to accept other duties needed or assigned by the resident or institution.

JOB SUMMARY:

The Wound Nurse is responsible for the coordination of wound care for the residents at the LECOM Institute for Successful Aging. These include, but are not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through standard operating procedures of all wounds.

MINIMUM QUALIFICATIONS:

• Must be Licensed as a Practical Nurse or Registered Nurse in Pennsylvania.
• Wound Care Certification or Ostomy Certification is preferred but not necessary.
• 1-2 years related experience and/or training; or equivalent combination of education and experience.
• Must maintain current certifications in CPR and First Aid.

TYPICAL JOB DUTIES:

• Provide case management for consulting patients providing specific assessment according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patients’ wound care;
• Provides efficient and effective care with the guidance of the Physician during the wound clinic sessions, attends or follows up on all wound clinic appointments where scheduled. Able to manage a high number of patients at the same time with high energy level and organization;
• Collaborates with other health care providers, wound care Physicians, Nursing Leaders and floor staff, Home Health Agencies, Extended Care Facilities, and Wound Clinic Program Director and Clinical Coordinator, regarding resident needs;
• Will participate in the development and establishment of a goal oriented skin care protocol;
• Implementation of the skin care protocol and orders within 24 hours or next business day after admission;
• Order entry of initial LECOM Wound Clinic orders;
• Order entry as needed upon changes in skin care;
• Daily review of skin incidents and wound / ulcer report accuracy;
• Liaison to LECOM Health Wound Clinic for transfer assistance;
• Assist in resident / family / employee teaching and education as needed;
• Review , document and report significant changes as well as quarterly and annual care plans;
• If Patient has an active wound, will participate in multi-disciplinary team care plan meetings;
• If Patient has no active wounds, a wound nurse business card will be provided during the care plan meeting;
• Must provide daily skilled wound care on all patients, as applicable;
• Nursing staff will perform head to toe skin assessments upon admission or next business day after admission and communicate results to wound nurse;
• The wound nurse will assess either upon admission or on next business day after admission if there is any presence of a wound;
• The wound nurse will assess within 5 days of an admission if no wound is identified by the nursing staff in their head to toe assessment;
• and continue performing at least weekly skin assessments;
• Weekly wound care rounds, follow up and documentation.

The Project Manager is responsible for developing, overseeing, and improving The Graduate Medical Education Residency Program under the direction of the Program Director, implementing changes based on the current accreditation requirements, and preparing for accreditation site visits and review by the ACGME Review Committees.

The ideal Candidate will possess prior Healthcare or Project Management experiences who have the ability to organize materials, scrutinize and interpret regulations and guidelines to create and improve programs and directives, however previous GME coordination experience is not required.

• Provide administrative support to Program Directors, GME Administration, residents and fellows as needed across all programs
• Assist with documentation for resident and fellow training, including but not limited to scheduling meetings, problem solving, and maintaining program and institution files
• Assist in ACGME credentialing process for residency and fellowship, i.e. gathering information for Program Information Forms
• Prepare and assist with all aspects of accreditation requirements, site visits and inspections
• Track and maintain aspects of Residency, including:
• Collection of Goals & Objectives forms
• Assist with data entry of completed reporting forms, evaluations and documentation
• Schedule Resident/Faculty Workshops, meetings and facilities
• Serve on the Orientation Planning Committee
• Keep various schedules and calendars up to date and review appointments as needed with program leadership
• Attend and record minutes at various Committee meetings, and compose agendas as needed, including GME, GMEC, and patient safety
• Maintain program organizational files
• Coordinate all necessary certifications for incoming and existing trainees, including but not limited to BLS, PALS and ACLS
• Complete requests for credentials and verification of training forms
• Assist with licensure
• Facilitate Resident/Fellow membership in appropriate organizations as well as subscriptions/generate user accounts as needed
• Maintain and appropriately communicate the Program Director’s schedule
• Schedule and coordinate meetings and conference calls
• Coordinate the Program Director correspondence

Qualifications

• Associate's degree in a business related field, required

LECOM Institute for Successful Aging is currently seeking a Full Time LPN for one of our personal care facilities – the Regency at South Shore, located at 322 Washington Place, Erie, PA.

Summary:

• Responsible to participate in hiring, training, scheduling, and supervising staff.
• Responsible to conduct and document training programs including but not limited to initial training, annual training, monthly training, diabetes training, CPR and first aid training.
• Responsible for the resident medications including training on medication administration and management.
• Responsible to serve on call after office hours.
• Responsible for the assessment, care, and monitoring of residents and the related documentation.
• Responsible to have positive relationships and communication with families.
• Communicate with physicians.
• Responsible to follow nursing practices and documentation according to Pennsylvania regulations

Qualifications Include:

• Must possess current Licensed Practical Nurse (LPN) in Pennsylvania.
• Must maintain current certifications in CPR and First Aid.

The LECOM Institute for Successful Aging is now accepting applicants for its Certified Nurse Aide course.

Applicants selected for the LECOM CNA course will receive their training at no cost, as well as books for the class. LECOM will also cover the cost for the licensing exam (terms apply).

Applicants will be offered a Full Time CNA position within one or more of our skilled nursing facilities, contingent upon successful completion of class and the final exam.

About the Class
The LECOM CNA Course is a 4 week course, with a total of 120 hours of classroom and clinical training. The first two weeks of the course will be classroom hours hosted at the Regency at South Shore, located at 322 Washington Place, Erie PA 16505, from 8:00 AM – 2:45 PM. Clinical hours will be held during the second two weeks of the course at the LECOM Senior Living Center, located at 5535 Peach St, Erie PA 16509, from 7:00 AM- 1:30 PM. Candidates who can work Full Time 2nd shift, 3rd shift, or 12 hour overnight shift upon graduation of the course will be given preference for admittance. Attendance is during the class is mandatory.

Qualifications

• High School Diploma or GED equivalent, strongly preferred
• Applicants must complete a pre-employment physical and criminal background check prior to beginning the class
• Must have reliable transportation to and from the class location(s)

JOB SUMMARY:

The Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services while operating in a resident centered environment.

TYPICAL JOB DUTIES:

• Completes an initial comprehensive psychosocial assessment of patients.
• Must be an advocate for the resident to resolve crises.
• Collaborates with other professionals to evaluate the physical, mental, and social aspects of the resident.
• Monitor, evaluate, and record residents’ progress according to measurable goals described in treatment and care plan.
• Modify treatment plans to comply with changes in residents’ status.
• Identify environmental impediments to the residents’ progress through interviews and review of resident records.
• Counsel residents in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
• Educate residents and family members on the roles and levels of healthcare, community resources, entitlements, and advanced directives.
• Arranging for resources/funds to finance medications, durable medical equipment, and other needed services.
• Coordinates resident discharge and continuity of care planning.

MINIMUM QUALIFICATIONS:

• A bachelor’s degree in a Human Services field including, but not limited to sociology, special education, rehabilitation counseling, or psychology from an accredited institution.
• One year of supervised social work experience in a health care setting working directly with individuals.
• Must meet equivalent state requirements for social work practice, or, in jurisdictions not having such legal regulation, holds certification or credentialing from the National Association of Social Workers.
