Previous healthcare maintenance experience a plus but not necessary.
LECOM at Village Square is located at 149 W 22nd St, Erie PA.
The Maintenance and Housekeeping Supervisor performs a variety of semi-skilled maintenance work while supervising housekeeping staff. Will be responsible for knowledge of and adherence to healthcare regulations, while maintaining safe and operating condition through preventive maintenance programs and providing guidance to approved maintenance vendors.
*This position may require travel between multiple buildings/locations within Erie County while working. A valid driver's license and reliable transportation are required. A driver record check will be conducted for the chosen candidate as part of the onboarding process.*
QUALIFICATIONS:
- High School diploma and/or equivalent required.
- Experience with Skilled Nursing and/or Long Term Care Facilities, as well as Dept of Health requirements within those facilities is strongly recommended.
- Job related experience within specialized field with two to five (2-5) years’ of experience required.
- Two years of verifiable paid experience in the skilled trades and/or construction or maintenance field or, electrical, plumbing, locksmith or HVAC trade; or in leading other maintenance workers, with concentration in a skilled craft.
- Valid Driver’s License and Evidence of Insurability

JOB DUTIES:
- Collaborates with site personnel, other trades, subcontractors (e.g., scheduling work orders and work activities, identifying project specifications, estimating costs, establishing required delivery dates of equipment and supplies) for the purpose of providing written support, conveying information, and coordinating work assignment activities;
- Coordinates with administration and other trades for the purpose of completing projects/work orders efficiently;
- Guides other maintenance workers in performing skilled crafts for the purpose of providing support to workers developing their knowledge of specific skills with trade; Inspects facilities, systems and their components for the purpose of ensuring safety (e.g. OSHA) and cleanliness, and identifying necessary repairs;
- Installs a variety of items and/or system components frequently of a complex nature (e.g., cabinetry, doors and hardware, windows, playground equipment, classroom and office equipment and facility components, TV cables, audiovisual equipment, intercommunicating telephones) for the purpose of maintaining the Institution in a safe, comfortable and operating condition;
- Maintains tools and/or equipment for the purpose of ensuring the availability of equipment in safe operating condition;
- Performs a variety of facility and equipment repair and maintenance activities at multiple sites within areas of trade expertise (e.g., electrical, heating/air conditioning, carpentry, plumbing, heating, locksmithing, roof and rain gutters, concrete, drywall, stucco, painting) for the purpose of ensuring that Institutional facilities and equipment are maintained in a safe and effective working order;
- Prepares and maintains a variety of documentation (e.g., work orders, repair records, purchase orders, requisitions) for the purpose of providing written support and/or conveying information;
- Procures equipment and supplies in collaboration with onsite and outside vendors for the purpose of maintaining inventory and ensuring availability of required items;
- Responds to emergency situations for the purpose of resolving safety concerns and taking appropriate actions to maintain the overall educational environment;
- Tests and/or troubleshoots a variety of items dependent on specific trade expertise (e.g., thermocouples, motors, boiler controls, motor valves, fire alarms, plumbing valves, fixtures, water lines, alarms, locks, new and renovated construction) for the purpose of assessing item functioning and recommending repair or replacement;
- Assigns workers their duties and inspects work for conformance to prescribed standards of cleanliness;
- Investigates complaints regarding housekeeping service and equipment, and takes corrective action;
- Obtains list of rooms to be cleaned immediately and list of prospective check-outs or discharges to prepare work assignments;
- Coordinates work activities among departments;
- Conducts orientation training and in-service training to explain policies, work procedures, and to demonstrate use and maintenance of equipment;
- Inventories stock to ensure adequate supplies;
- Evaluates records to forecast department personnel requirements;
- Makes recommendations to improve service and ensure more efficient operation;
- Prepares reports concerning room occupancy, payroll (Kronos), and department expenses;
- Selects and purchases new furnishings;
- Performs cleaning duties in cases of emergency or staff shortage;
- Examines building to determine need for repairs or replacement of furniture or equipment, and makes recommendations to management;
- Attends staff meetings to discuss Institutional policies and patrons' complaints;
- Issues supplies and equipment to workers;
- Establishes standards and procedures for work of housekeeping staff;
- Advises manager, desk clerk, or admitting personnel of rooms ready for occupancy;
- Records data regarding work assignments, personnel actions, and time cards, and prepares periodic reports;
- Screens job applicants, hires new employees, and recommends promotions, transfers, and dismissals; and
- Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning of the work unit.

Under the supervision of a Physical Therapist, the Physical Therapy Assistant administers medically prescribed physical therapy treatment for patients suffering from injuries and muscle, nerve, joint and bone disease, to restore function, relieve pain, and prevent disability by performing a numbered of required duties for the LECOM Institute for Successful Living.
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Review physician orders, patient‘s condition, and medical records;
- Review data from the Physical Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
- Review effective treatment strategies for areas of strength, endurance, and functional mobility with the Physical Therapist;
- Monitor and communicate patient response to physical therapy treatment;
- Assist the Physical Therapist with patient at discharge including patient and family instruction in safety, mobility and function;
- Attend meetings in regards to patient care such as care plan meetings , welcome meetings and daily huddles on an as needed basis;
- Modify physical therapy treatment and plan of care according to patient’s ability and response as directed by the supervising Physical Therapist;
- Instruct patient and family in treatment procedures to be continued at home;
- Record treatment, response, and progress in the electronic medical record. Documentation of patients care and services will adhere to accepted regulatory, corporate and professional guidelines;
- Record daily treatment charges per corporate procedures’
- Adhere to all HIPPA procedures and guidelines in accordance with LECOM Health’s standard of excellence;
- Educate the patient and/or caregivers to enhance functional independence;
- Comply with federal, state and local legal and professional requirements;
- Maintain compliance with appropriate therapy minutes, therapy intensity, and productivity standards on daily basis;
- Work as scheduled, ready to devote full attention and energy to the important work of LECOM Health; and
- Accept other duties as needed/assigned for the Institution’s needs.
MINIMUM QUALIFICATIONS:
- Physical Therapy Assistant degree from an accredited institution required.
- Knowledge of OSHA regulations, CDC guidelines, hazardous waste management and disposal and radiation safety.
- CPR and Pennsylvania license as a Physical Therapy Assistant required.
- Must also possess and independent supervision license.
- Valid Driver's license

The Assistant Director of Nursing is a Registered Nurse (RN) who will be responsible for the day to day coordination and oversees all aspects of the Nursing Department as assigned by the Director of Nursing. - Provides and facilities resident centered care.
- Performs routine skin evaluations and ensures the treatments are ordered and applied appropriately.

- Assists DON in aspects of marketing including, but not limited to, developing positive relationships with the community, residents, and families. Providing tours and attending marketing events will also be required.
- Assists in the negotiation of approval for resident stay and rates with payers.
- Serves as an educator for residents, families, referral sources, payers, and case managers.
- Participate in precertification, preadmission screenings, outcome data, and review of records.
- Assists in the coordination of admissions and discharges.
- Conducts resident interviews for social, personal, and medical history.
- Assists residents with the activities of daily living including, but not limited to; bathing, dressing, dining, and transportation.
- Assists in the care planning process including implementation and accurate and timely documentation.
- Will perform as the Acting or Interim D.O.N. in the absence of the D.O.N.
- Must be flexible to accept other duties as needed or assigned by the resident or institution.
- Performs other duties as assigned by supervisor or management.

Job Summary:
Provides direct and indirect patient care in the emergency care setting by contributing to all steps of the nursing process (assessment; planning; implementation and evaluation). The LPN shall communicate with the RN and seek guidance when the patient’s care needs exceed the LPN’s scope of practice or surpass the LPN’s knowledge, skill or ability. The LPN will also communicate to the RN and/or ED Physician if the patient’s condition deteriorates or there is a significant change in condition. The LPN will notify the RN and/or the ED Physician if the patient is not responding to therapy or the patient becomes unstable and/or the patient needs immediate assistance. The LPN shall obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. The LPN should be able to triage safely, rapidly and accurately. The LPN will provide care that reflects initiative, flexibility and responsibility indicative of professional expectation with a minimum of supervision. The LPN will determine priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. The LPN will be able to respond quickly and accurately to changes in condition or response to treatment.
Characteristic job tasks and responsibilities:
- Knowledge of medications and their correct administration based on age of the patient and their clinical condition.
- Ability to perform a head-to-toe assessment of all patients and reassessments as per policy recognizes changes in condition and report findings accurately to the RN. This includes infant/pediatric and general patient population.
- Rapidly and accurately assigns triage categories utilizing the Estimated Severity Index, Level 1-5.
- Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
- Communicates with the Emergency Department RN and physician about symptomatology and results of diagnostic studies.
- Communicates appropriately and clearly to physicians, RNs, and other co-workers.
- Revises standardized ACI based upon identified learning needs and evaluates effectiveness of learning while including family as appropriate.
- Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
- Ability to function well under stressful situations.
- Demonstrates an ability to assist physicians with procedures in the Emergency Department.
- Treats patients and their families with respect and dignity. Assists manager in the development, implementation and evaluation of QI indicators to improve patient outcome and work environment.
- Documentation meets current standards and policies.
- Must attend all required staff meetings.
- Performs other duties as directed by Registered Nurse.

Under supervision of an Occupational Therapist, the Occupational Therapy Assistant administers medically prescribed occupational therapy treatment for patients suffering from injuries and muscle, nerve, joint and bone disease, to restore function, relieve pain, and prevent disability by performing a numbered of required duties for the LECOM Institute for Successful Aging, and other LECOM Health entities, in accordance with current applicable federal, state and local standards, regulations, and guidelines to assure that the highest degree of quality care is rendered and LECOM Health’s standard of excellence is achieved.
CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:
- Review physician orders, patient‘s condition, and medical records;
- Review data from the Occupational Therapy evaluation and plan of care including short and long term goals prior to administering treatment;
- Review effective treatment strategies for areas of Activities of Daily Living and Independent Living Skills with the Occupational Therapist;
- Monitor and communicate patient response to occupational therapy treatment;

- Assist the Occupational Therapist with patient at discharge including patient and family instruction in safety, mobility, and function;
- Modify Occupational Therapy treatment and plan of care according to patient’s ability and response as directed by the supervising Occupational Therapist;
- Instruct patient and family in treatment procedures to be continued at home;
- Record treatment, response, and progress in the electronic medical record. Documentation of patients care and services will adhere to accepted regulatory, corporate and professional guidelines;
- Maintain compliance with appropriate therapy minutes, therapy intensity, and productivity standards on daily basis;
- Adhere to all HIPPA procedures and guidelines in accordance with LECOM Health’s standard of excellence;
