LECOM Health offers a full continuum of care from Independent Living and Personal Care apartments to Skilled Nursing Care and Outpatient services including In-Home Care, Home Health, and Hospice under the direction of the LECOM Institute for Successful Aging.

The Institute for Successful Aging is currently seeking Full and Part Time Housekeepers in Erie, PA to perform duties at either one or more of the facilities within the Institute.

Per Diem employment options may also be available.

Summary:

Under general supervision, performs a wide variety of housekeeping duties focusing on cleaning techniques in order to provide a clean, orderly and safe environment; and perform related work as required.

The LECOM Institute for Successful Aging is currently accepting applications and resumes for a Full-time Cook to work at one or more of our senior care facilities.

Summary:

  • The cook will prepare bulk food items for the various venues while following recipes to be nutritious, attractive, in proper portions while being responsible with resources. Will also be responsible to report problems and take corrective action to address those issues.
  • Possesses knowledge, skills, attitude, and energy to consistently provide resident centered care.
  • Maintains and promotes ethical conduct of employees, addresses residents in a friendly manner, creating a safe and productive environment.
  • Will manage the schedule, food supply and food production duties in the absence of dietary managers.
  • Responsible for daily food preparation to prepare food for multiple venues while following recipes.
  • Responsible for food production for catering events, off-premises food services including meals on wheels, LECOM Center for Health and Aging, and other customers as assigned.
  • Promotes a safe working environment.
  • Read and interpret the Resident’s nutritional requirements via paper or digital platform.
  • Promote clinical nutrition through the Dietitian’s management of therapeutic diets, nutritional supplements and the food first philosophy while producing accurate food offerings.
  • Educates and guides residents in regards to cost and contents of their selections and provides alternative selections available.
  • Ensures health guidelines and company sanitation policies are followed by employees and guests in all dining areas.
  • Maintain all documentation and logs for sanitation and food safety compliance purposes.
  • Assists the Dietary Manager to facilitate departmental auditing in preparation for annual survey.

Job Description:

This PGY-2 geriatric pharmacy residency program offers an opportunity to excel in clinical and consultant geriatric pharmacy via interactions with a rich health care community. Learning experiences cover a wide array of geriatric experiences and include: acute care, ambulatory care (longitudinal), long term care (core rotation and longitudinal), inpatient rehabilitation, public health, healthcare administration, academic/teaching, and hospice/palliative care. Electives are available in critical care, infectious disease, and other areas. This program offers a flexible schedule designed to give the resident additional experience in their specific area of interest. The resident will gain experiences in teaching and an optional Medical Masters of Science in Medical Education (MSMed) degree is also offered.

Duration:

July 1st 2024- June 30th 2025

Site Special Features:

The PGY-2 residency program is in association with a school of pharmacy, a college of medicine, the LECOM Institute for Successful Aging, and with a community/teaching hospital (medical residency/fellowship programs). We are located at the border of Lake Erie, a small town with the amenities of a larger city, two hours from Cleveland, Buffalo, and Pittsburgh.

Qualifications:

  • Doctorate of pharmacy degree
  • Completion of ASHP accredited PGY1 pharmacy residency
  • Copy of PGY1 residency certificate presented to residency program director
  • Pennsylvania pharmacy licensure (or eligibility for licensure by July 31st, 2024)
  • Three letters of recommendations
  • Curriculum vitae
  • Pharmacy school transcripts

Responsibilities:

  • Provide clinical services and comprehensive medication management to patients during all core and longitudinal rotations
  • Ensure continuity of care during transitions of care between settings
  • Manage and facilitate delivery of medications
  • Perform comprehensive medication regimen reviews monthly on a subset of patients in the long term care setting
  • Demonstrate leadership skills via interactions with pharmacy technicians, pharmacy interns, and pharmacy students
  • Effectively provide education to patients, caregivers, pharmacy students, and the interprofessional healthcare team
  • Complete a quality improvement research project and medication use evaluation with poster presentation at an annual pharmacy assembly

The Clinical Liaison is a facilitator to all departments and units within the organization in relation to admissions and providing a continuum of care. The Clinical Liaison is an educator, advocate for the patients, consultant, marketer, negotiator and collaborator.

CHARACTERISTIC JOB TASKS AND RESPONSIBILITIES:

  • Collaborates with internal and external case management teams to provide a cost-effective plan for each client.
  • Responsible for interviewing patients and family members to obtain information regarding home environment, family relationships, health history, and personality traits of the patient
  • Serves as a rehabilitation consultant, considering rehabilitation potential, therapy progress, internal admissions teams, discharge plan, and special equipment needs.
  • Manages departmental policies and procedures, Quality Assurance program, safety, and environmental and infection control standards.
  • Provides available resources and education regarding utilization services to clients, family members, and referral sources and external payers.
  • Provides necessary information to facilitate referrals and subsequent admissions.
  • Negotiates approval for a client’s stay and rates with payers on necessary referrals.
  • Manages and completes all required documentation for preadmission screening.
  • Performs on site screenings or out of building record reviews, including telephone preadmission screenings and clinical assessments.
  • Assists the Director of Behavioral Health with the departments marketing efforts and facilitating referrals.
  • Coordinates all transitions from the referring facility or site in a professional and timely manner.
  • Refers and recommends appropriate care facilities for candidates who are denied admission.
  • Conducts other duties, such as marketing events and providing tours.

MINIMUM QUALIFICATIONS:

  • A Nurse, Respiratory Therapist, Social Worker, or skilled clinician, preferred but not necessary
  • Familiarity with the resources available in assessing regulations and the parameters of third-party reimbursement a plus.
  • Excellent assessment skills and understanding of the insurance and managed care arenas.
